On February 24, 2026, at 8:40 A.M., Licensing Program Analyst (LPA) Joselito L. Del Mundo, conducted a Pre-Licensing inspection with applicant, Melissa Marie Taylor to ensure the facility meets basic licensing requirements set forth by Title 22, Health and Safety and statutory requirement. The licensee recently relocated and is requesting to provide care for a Large family childcare home for the capacity of 14 children at this new location. Currently living in the home are the applicant, one adult (Applicant’s spouse), and four minor children (17, 12, 12, and 8). LPA toured the home indoor and out to ensure it meets FCCH requirements. The applicant is requesting to provide care for fourteen children, ages zero old to eighteen years of age.

Based on the Fire Safety Inspection Request, fire clearance was granted for LDFC to house (14) ambulatory children. External address numbers added as requested. Water heater cleared of all storage to 3’ plus as requested. External gate release updated as requested. No violations noted on reinspection. Children to be cared for in areas discussed and permitted for R-3 occupancy only.

Applicant is also requesting the days and hours of operation will be seven days a week, from 6:00 A.M. to 4:00 A.M.

The home is described as follows:

This is a two-story home with five bedrooms (5 upstairs), three bathrooms (2 upstairs and 1 downstairs), living room #1, living room #2, kitchen, dining area, garage, and back yard. The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons,

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Living room #1: LPA observed couches, a vacuum cleaner, lamp stand, television, and wooden furniture.
The outlets are covered.
Living room #2 (Main care): LPA observed couches, small table and chairs, computer table, television, furniture and fixtures, and a fireplace. The fireplace is covered by a mesh screen, and it is also blocked by an iron gate. The outlets are covered. LPA observed toys in the living room.
Dining area: LPA observed dining table and chairs, printer, 3 highchairs, water dispenser, and cabinets for storing food and other children’s supplies. The cabinets have locks. The outlets are covered.
Bathroom #1: LPA observed toilet, shower, sink and are in operable conditions. LPA did not observe razors and/or sharp objects, shampoos, mouthwash, medications, perfumes, air freshener, nail polish and nail remover that will pose a health and safety risk to children in care. Cleaning supplies and other detergents are stored on the top shelf of the bathroom cabinets. The cabinets have child safety latch. LPA also observed a laundry area in the bathroom which contains washer and drier. The outlets are covered.
Covered Patio in the Backyard (Outdoor play area): Per applicant, she does not have body of water in the backyard. LPA did not observe body of water in the backyard. LPA observed a covered patio which will serve as the outdoor play area. LPA also observed a raised concrete flatform and a barbeque grill that is mounted on bricks. LPA observed a small basketball hoop, table, chairs, and other toys inside the storage box. The child safety baby gate also enclosed these structures.
Electrical outlets: Outlets in the on-limit areas have child safety plastic plugs.
The stairs leading to upstairs room have a child safety baby gate.

Bathroom #2 (Located between bedroom #2 and bedroom #3): LPA observed toilet, shower, and sink and are in operable condition.
Bedroom # 3 (Adjacent to bedroom #4): Bedroom #3 was equipped with a bunk bed, video arcades, television, drawer, closet, and other personal belongings.
Bedroom # 4 (Adjacent to bedroom #3): Bedroom #4 was equipped with a twin-size bed, drawers, closet, television, and other personal furnishings.
Bedroom # 5 (Master bedroom): Bedroom #5 was equipped with a Cal-king sized bed, dressers, drawers, closet, and other personal furnishings. Bedroom #5 has its own bathroom (bathroom #3).
Bathroom #3: LPA observed toilet, shower, faucet, and 2 sinks and are in operable condition. The medicine cabinet contains household medications.
Kitchen (Adjacent to dining room and living room #2): The kitchen has 2 child safety baby retractable mesh gate. LPA observed a stove, refrigerator, dishwasher, microwave oven, and other kitchen equipment. The outlets in the kitchen have child safety covers. The knives are stored in the upper cabinets above the stove. Cleaning compounds were observed to be stored inside the lower kitchen cabinets under the sink and the cabinets have child safety latch and magnetic locks. Per applicant, meals and snacks will not be provided to children in care. LPA told the applicant that if food was brought from children’s home, the container shall be labeled with the child’s name and properly stored or refrigerated.
Garage: The garage contains refrigerator, freezer, personal computer, and other household items. Per applicant, the garage is also considered to be the mancave.
Backyard and side yard: LPA also observed trees, plant boxes, turf grass, a dog inside the dog run, tables and chairs, and two tool sheds. LPA observed the toolsheds have pad locks. The tool sheds contain household items. The applicant was advised to have the tool sheds locked during day care operations. On the side of the house, there were two iron gates (above 5 feet in height) and a small iron gate. Per applicant, the small iron gate is used as an emergency exit. LPA also observed trees, plant boxes, barbeque grill, old tires, and other household items.
Napping equipment: LPA observed eight sleeping mats and 2 foldable cribs (wooden and mesh).
Weapons or Firearms: Per applicant, there are zero firearms or weapons. LPA did not observe any weapons or firearms.
Knives: Stored inside the upper kitchen cabinet above the stove.

Detergents and Cleaning compounds: Cleaning compounds were observed in the bathroom cabinets and in the lower cabinets of the kitchen.
A smoke detector and a carbon monoxide detector: Tested at 10:30 A.M. Carbon monoxide detector and smoke detector are in operable condition.
Fire extinguisher (2-A:10-B:C): There is a 2-A:10-B:C type of fire extinguisher.
First Aid Kit: LPA observe the first aid kit with emergency guide, tweezers, scissors, and other emergency supplies. Applicant has a non-contact thermometer.
Pets: LPA observed one dog (pets) inside the dog run in the backyard. Applicant provided a copy of the vaccination records for the dog.
Medications: Stored in the licensee’s room and in the upper kitchen cabinets
Documentation:
Applicant completed the Prevented Health and Safety Training with the nutrition and lead component on December 8, 2020. Applicant completed the pediatric CPR and First Aid Training on March 8, 2025, and mandated reporter training on January 10, 2025. Applicant and one adult living in the home were fingerprint cleared. Applicant has the required immunizations, Pertussis and Measles. Applicant declined the Influenza vaccine. Applicant and the other adult living in the home have TB skin test. During this visit, the applicant also provided the LPA with a copy of the letter stating that the applicant no longer resides on the address where applicant was previously providing day care.

Per Applicant, transportation will be provided.
The following was discussed with the applicant:
1. The following items are zero tolerance by Licensing: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
2 Pediatric First Aid and CPR: American Heart Association or American Red Cross or Emergency Medical Services Authority (EMSA) approved in Pediatric First Aid and CPR must be present. Certification must be renewed every two years.
3. Applicant was informed that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

4. In the absence of the licensee a qualified adult must be present, supervising the children; a qualified adult is an individual who has a valid and current Pediatric first aid/ CPR-adult-child- infant certification (EMSA approved), a valid criminal record clearance associated to the facility license, immunization's (MMR, TDAP, TB and Influenza or Influenza declination), AB 1207 Child Abuse Mandated Reporter Certificate.
5. A current roster of children enrolled must be available and maintained for a period of 3 years, even after children are no longer attending the facility.
6. Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the license shall be terminated
7. The fire extinguisher type 2-A:10-B:C must be serviced annually or as often as necessary and smoke and carbon monoxide detectors should be checked, and batteries replaced as needed.
8. Fire and safety drills must be performed every six (6) months and documented for review by the Department.
9. Smoking is prohibited in the family childcare home.
10. Children and staff records must be maintained and updated as needed and be available for review by the Department.
11. Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles.
12. Inspection Authority: All adults living and working in the home shall be made of aware of the Department’s right to inspection the home, which includes, but is not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.
13. The facility license number must be on all advertisements, publications, or announcements with the intent to attract clients.
14. Isolation for Ill children: When a child is ill, he/she shall be separated from other children (reference 102417(e) Operation of a Family Child Care Home). The living room #1 will be use as an isolation area.
15. Liability Insurance was discussed; LPA advised applicant to review Title 22 Regulation 102417(m)(1) for additional information. Currently, Licensee has no plans of getting a liability insurance.
16. Dog(s) and/or pets are recommended to be isolated from children in care.
17. No baby bouncers, no infant walkers, no Johnny jumpers, no saucer chairs, and any other item that falls into this category is not permitted in the facility.

18. The licensee shall be present in the home and shall ensure that children in care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day.
Applicant, Melissa Taylor was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The applicant, Melissa Taylor provided proof of control of property.

LPA discussed the safe sleep regulations with applicant, Melissa Taylor and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, February 23, 2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law.

No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant, Melissa Taylor was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms.

A notice of site visit was given to applicant, Melissa Taylor and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

As a result of this inspection, the home does not meet Title 22 Regulations. Final approval is pending the following:

1.Physical Plant – 102417(g): Applicant will install a child safety cover for the barbeque grill mounted on the bricks and on the small door of the bricks that contains other supplies.



Applicant will send photos of the corrections on February 24, 2026, to LPA.
