On 5/16/2023 at 11:00am Licensing Program Analyst (LPA) V. Wheatley met with owner/director Gyoungok Park regarding the above allegations. LPA was guided on a tour of the school age program. LPA observed 9 school aged children on the premises after they arrived after 1:30pm. The children were picked up by owner's husband Yunseok Lee. LPA interviewed witnesses regarding the above allegations.

LPA inspected the kitchen and observed food that should be refrigerated on the counter. In addition, LPA observed spoiled vegatables in the refrigerator. The staff serve the children food.

LPA inquired staff about the fire drills. Director states disaster drills are not being conducted and does not have proof to show the fire or earthquake drills are being conducted.

Based on the information obtained through LPA's observations and interviews conducted, the allegations are substantiated. A substantiated finding means that the complaint is substantiated and the allegation is valid because the preponderance of the evidence standard has been met. A type A violation will be provided to all parents.

An exit interview conducted. A copy of the report will be provided.

The director will clean two refrigerators and discard all old and spoiled food immediately. The director will ensure the food is healthy and not expired or spoiled. The director will submit a plan of correction to the Department by 5/23/2023.

LPA inspected the kitchen and observed old food (vegetables) in the refrigerator which was displaying progressive growth. In addition, LPA observed meat on the counter which should have been refrigerated and per director rice from yesterday. This is a potential risk to the health and safey of children in care.

The director will clean two refrigerators, oven, mircowave, counter tops and sinks. The director will ensure the kitchen stays clean at all times. The director will submit a plan of correction to the Department by 5/23/23.

The owner/director will ensure that all children are supervised at all times. The owner/director will make sure that enough staff are hired to supervise all children. The licensee will enroll in a substituate program that will allow substitute teachers to work on the premises. The licensee will submit a plan of correction to the Department by tomorrow 5/17/23 explaining how she plans to stay in compliance.

This is evidenced by:LPA conducted interviews with witnesses and learned that the school aged children have been left unsupervised on more than one occasion. This is an immediate risk to the health and safey of children in care. A civil penalty will be assessed.
