On October 29, 2021 at 9:28AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an unannounced inspection on the above allegations. LPA disclosed the purpose of inspection and was granted entry by Staff 1. Upon entry LPA was greeted by Director who guided LPA on a tour of the faciltiy. Upon entry, LPA counted 0 children in care as it was prior to school being out of session for the day.

During investigation, LPA conducted interviews with staff, parents, and children. Based on evidence obtained, the facility staff notified staff and parents of the positive COVID-19 case and closed the facility to quarantine for 10 days as recommended by the Department of Public Health. Facility staff underwent further COVID-19 tests as a safety precaution, which were disclosed as negative.

It was also disclosed the facility staff are required to wear masks at all times and are required to complete a symptom survey prior to entry into the facility on the Brightwheel Application. Children are also required to wear masks at all times excluding outdoor play, eating, and napping.

LPA observed children wearing masks while indoors and remove masks during nap time, outdoor play, and lunch. LPA also observed staff assist children in replacing masks when soiled and providing friendly reminders to children to place the mask above the nose while wearing a mask.

Additionally, interviews indicated staff working at the facility are all of age and the individuals children listed as staff were Eligible-Cleared on association list. LPA did not observe staff working at the facility missing from the association list during inspection. By the time of this report, Staff 1 had completed the required fingerprint clearance process and was also Eligible-Cleared on association list.

The facility staff provide transportation for the children with parent approval to and from school, to the facility, and home. Based on interviews, children are not taken to any additional locations outside the approved locations.

Interviews also concluded the children have not been exposed to or have experienced bullying by other children. Staff disclosed they are very quick to respond when confrontation occurs between children and children did not disclose any instances involving being bullied by other children or being struck by older children.

Based on evidence obtained and interviews conducted, the above allegations are deemed Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations did or did not occur.

An exit interview was conducted, a copy of this Report, Appeal Rights and Notice of Site Visit were provided to Director.
