25 | On 6/20/2024 at 8:36am, Licensing Program Analyst (LPA) Andrea Pittman conducted an unannounced case management visit at the facility and was met by facility staff who permitted entry to the facility. Upon arrival, the Director Loannie Ramirez was providing coverage in the infant classroom with 3 infants. The LPA was later met by the Director Loannie Ramirez who took over control of the visit from the staff. LPA toured the facility according to the facility sketch. The Licensee Yesenia Canchola took control of the visit from the Director and the LPA finished the visit with the Licensee. LPA observed 8 infants with 3 staff members providing care and supervision, including the Director.
LPA conducted a visit to acquire pertinent documentation and conduct interviews with relevant parties; however, two of the staff that the LPA needed to interview are no longer working for the facility, the Director provided their contact information. LPA discussed the cleanliness of the classroom and that the floors were clean. However, LPA observed at 9:01am that the floor mats were mostly dirty that the infants were playing on, the Director stated that the mats are mopped daily at the end of the day and pressure washed over the weekends but the mats showed visible signs of being soiled; at 9:02am various infant toys and a child seat showed signs of soil, as well, the Director stated that they are supposed to be cleaned daily by the teaching staff. At 9:08am, the LPA observed a bug crawling on the floor of the indoor activity area where the infants were playing, the Director promptly removed the bug, there was no other signs of vermin on the floor in the indoor activity space. Technical assistance was provided on the infant sleeping area being separate from the indoor activity area, director qualifications, and policy. The Licensee was explained the observations made by the LPA and the Licensee stated the changes that will be made to address the deficiencies. |
