25 | On 7/10/2024, Licensing Program Analysts (LPA) Andrea Pittman conducted an unannounced case management visit at the facility and was met by the facility representative who permitted entry to the facility. LPA toured the facility with the facility representative according to the facility sketch. Upon arrival, LPA observed 7 infants with 2 staff member providing care and supervision.
During this inspection, LPA received pertinent documents related to this investigation which included the facility’s staff and children’s rosters, personnel records, and other relevant documents.
On 5/1/2024, at 11:10am, LPA Pittman and the Director entered the infant classroom. LPA Pittman observed in the infant classroom that there were infants forced to sit in the high chairs not eating or drinking and they remained in the high chairs until 11:45am. At 11:43am, LPA had to intervene and prevent a child that began climbing from the high chair from falling to the floor by catching them as they climbed out of the chair. This is a Type B violation, see the LIC 809D for the details. At 11:51am and 11:52am, LPA observed cleaning supplies that were out and accessible to children in care; during this time an infant climbed on the rocking chair and was able to access the cabinet behind the rocking chair where the cleaning supplies were stored on the cabinet and LPA observed cleaning supplies on another cabinet on the opposite side of the room. This is a Type B Violation, see the LIC 809D for the details. At 11:45am, after removing the infant from the high chair, Staff 2 placed the infant in the crib, the infant remained awake for over 20 minutes and did not want to sleep. The infant began to bang on the siding of the crib and cry. LPA informed staff that the infant cannot be forced to remain in the sleeping area if they did not want to go to sleep. At 11:26am and 11:44am, LPA observed two infants sleeping in the crib feeding from bottles while asleep in the crib, LPA inquired of staff do the infants normally sleep in the crib while drinking bottles and the staff admitted that the infants did so daily. This is a Type B violation, see the LIC 809D for the details.
