On July 15, 2021 at 8:58AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an unannounced annual/random inspection. LPA disclosed the purpose of inspection and was granted entry by Director, Ashley Alva. Upon arrival LPA observed two classrooms in use with a total of 21 preschool children (11 in preschool classroom and 10 in toddler classroom) and 3 teachers on the premises. Facility has a toddler component kept physically separate from the preschool children. The hours of operation are 6:00AM - 5:30PM Monday - Friday. The center also has a School-Age Program (197750087). Incidental Medical Services (IMS) were discussed.

Furniture and equipment were inspected for age appropriateness and good repair. All rooms are clean and safe. Telephone service was verified. Heating, lighting, and ventilation are adequate. There are cubbies for children's belongings. LPA observed age appropriate toys and materials. Drinking water is provided to the children using individual/personal water bottles sanitized daily and a vertex water filter/dispenser hooked to the main water line. LPA observed a sufficient quantity of cots available for napping children. The bedding is stored separately and properly. The sheets and blankets are washed twice weekly and cots are sanitized daily. There is a fire extinguisher (serviced 5/06/2021), smoke detector and a carbon monoxide detector on the premises.

LPA inspected and observed one clean bathroom (2 toilets and 2 sink). Additional sink in preschool age classroom for hand washing. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. LPA observed staff during circle time, breakfast, and crafting.

Children are inspected for illnesses as they arrive by taking the child's temperature and conducting an overall wellness check. A review of medication policy indicated that prescription medication is administered only with parent's written permission. The Director administers medication and documents the dosage, date and time onto a log. Parents administer over-the-counter medication or prescription medication prior to arrival at the facility.

Outdoor play equipment was inspected for health, safety, cushioning material, good repair and age appropriateness. The play area has cushioned playground material and concrete. There is an area for rest (canopy). Water pitcher and filled water bottles are brought out during outdoor play. Play area was inspected and found to be free of hazards and inaccessibility to bodies of water. Separate playground schedule for school-age, preschool, and toddler children. Outdoor Waiver is posted in the lobby. Jungle gym is anchored to the ground. There is a bike path and other age appropriate toys. LPA observed Staff during outside play as children rode bikes.

There is a clean kitchen (off limits) with refrigerator, freezer, toaster oven, microwave oven and dishwasher. The facility provides breakfast, lunch, snacks (morning and afternoon) and small dinner. Allergy lists are posted kitchen and classrooms. LPA observed an appropriate amount of food and snacks. Staff prepared breakfast using gloves. The chemicals are kept separate from the food (in closet adjacent to the kitchen locked and inaccessible to children).

Sign in and out sheets were reviewed. The parent board was reviewed and has the required forms posted. Fire/earthquake drills current. Roster current and complete.

Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Personal rights of children were discussed.

Children's records and staff records were reviewed. Staff are certified in CPR/First Aid expire 04/04/2023. Note: The training must be taken through American Red Cross, American Heart Association, or through Emergency Medical Services (EMS) Authority approved trainers.

--Director advised before an individual can obtain a childcare license or work at a licensed childcare facility, they must receive a criminal record clearance or exemption from the Department’s Care Provider Management Bureau (CPMB).

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Center Director advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation (call within 24 hours and submit written report within 7 days) and on the form LIC624. Center reminded to review updates/regulations as well as quarterly updates on Department website. The "Notification of Parent's Rights" poster must be posted in an area of the home accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.
No deficiencies cited according to Title 22 Regulations. Consultation provided during the inspection. Exit Interview conducted and a copy of report read and provided to Director, Ashley Alva.
