On 4/14/2026 Licensing Program Analyst (LPA), Isabel Ortega met with Director, Lucinda Inganni and conducted an Annual Random Inspection. LPA toured and inspected the facility in accordance with the facility sketch. During inspection LPA observed 11 children in care and 2 staff providing care and supervision. Facility operates in one classroom.

An isolation area takes place in the main office. Children have the option for rest time and quiet time if needed, blue child size cots are provided. Age-appropriate sinks and toilets were inspected for availability and good repair. Toilets flush properly; Each restroom has adequate toilet paper, hand soap and paper towels available. There is a changing table in the restroom. Restrooms are cleaned, restocked of toiletries, disinfected, and sanitized daily by staff.

The furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and AC ventilation were evaluated. LPA observed individual storage for children's belongings.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The classroom consists of purchased water from Pure Life; (LPA observed 5 gallon purchased jugs in the staff room) a black water dispenser was observed in the classroom and staff room Facility has individual disposable cups available for children when needed. Children bring their own bottled waters, which may be filled with purchased water as needed. According to facility Director, the water was not sampled for lead. Facility is aware a letter from facility and documentation shall be requested and approved by the Palmdale Regional Office. A type B citation was issued for not following the written directives PIN 21-21.
Facility operates Monday through Friday from 8:00 a.m. to 5:30 p.m. Facility provides snacks for children as needed. Parents Provide breakfast, lunch and snacks. Parents also provide the fitted sheets and blankets, which are taken home every Friday for wash and returned on Monday. Facility provides the blue child size cots for each child enrolled.
Facility has all the required posted documents: Facility License, Notice of Parent's Rights Poster, Emergency Disaster Plan, Child safety car seat and seat belt and Earthquake Preparedness Checklist.

First Aid supplies, smoke, carbon monoxide detectors were observed to be operating properly LPA tested the carbon monoxide detectors at 10:45 a.m. The facility could not locate a fire extinguisher (2A10BC). According to the Director the fire extinguisher is serviced once a year by the church and it is usually by the door but does not know what happened to the Fire extinguisher. A technical violation issued.

Trash cans with tight lids were observed. Snacks are properly labeled, stored within expiration date. A mini refrigerator is clean and operates at the proper temperature. Classroom Temperature is at 67 degrees.

Cleaning supplies are keep out of reach of children.

Emergency Drills are conducted every sixth month last drill was conducted and documented on March 09, 2026, at 10:45 a.m. Food allergies are noted and posted in each classroom when allergies are present. Currently no children enrolled have reported food allergies or any medical conditions.

The outdoor play area was inspected and observed to be free of hazards, loose, or sharp objects. Equipment was inspected for safety, cushioning material, good repair, and age appropriateness. The play yard has adequate shade for rest. The playground is well fenced all around, and no bodies of water observed in the outdoor play area. According to facility there are no weapons or firearms at the facility and LPA did not observe any weapons or guns during the inspection.



Children's records were reviewed for completeness. Health History, Emergency contact and Medical Exams; Immunization Records are all in the children's file. The staff have been fingerprinted and associated with the designated license number.
Two of two staff files were reviewed and neither staff present could provide a valid current Pediatric or EMSA approved CPR/First Aid training certificate. At least one member of staff is required to have a valid Pediatric EMSA approved CPR/First Aid training certificate and renewed every two years.

Two staff out of two staff present did not have a current certificate for Mandated Reporter training both certificates expired on October 2025. Mandated Reporter training is required every two years.
The following Incidental Medical Services (IMS) were discussed.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. All staff are required to take the Child Care Provider Mandated Reporter (AB1207) training every 2 years. www.mandatedreporterca.com. Upon hired date all staff are required to be immunized and show proof of immunization records according to Title 22 regulations. All staff shall have copies of immunization on file- MMR, TDap, TB and influenza. Facility could not provide proof of MMR for one out of two staff. A type B citation was issued.

Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required.

Per Title 22 Regulations facility is not complying with rules and regulations, facility has been cited four deficiencies Type B today. Citations are the following Type B: Failure to have at least one staff Certified in Pediatric Or EMA approved CPR/First Aid, Expired mandated reporter training certificate two of two staff certificates expired 2025, One staff of two staff missing MMR vaccine and facility did not Sample water for lead- Written Directives PIN 21-21(Regulation waiver exempt not request). A Technical violation for no Fire Extinguisher on the facility. Facility was informed about Technical Support for Providers/Licensees(TSP).

An exit Interview was conducted with Lucinda Inganni, Director. A copy of this Report, a Notice of Site Visit and appeal rights were provided on this day. A copy of this report must be made available to the public for 3 years. The Notice of site visit shall be visibly posted for 30 days.
