This is an amended report for the report issued on January 09, 2024.

On January 23, 2024, Licensing Program Analyst (LPA) Joselito Del Mundo conducted a follow-up complaint inspection to Pure & Simple Academy Inc. LPA met with Director Deborah Castillo and Jovita Herrera. Soon after, Director Sandra Lona Aguirre came to the office. The purpose of the inspection was to deliver the findings regarding the above complaint allegation. The investigation included an inspection of the facility, a review of facility records, and confidential interviews with staff and parents. LPA observed 8 infants and 4 toddlers with 5 staff providing care and supervision.

On December 7, 2023, a complaint was received alleging Staff did not meet the infant's diapering needs resulting in diaper rash. Based on the information gathered, records revealed that C1 was being monitored and checked by the staff throughout the day according to the diaper log.

After observations and interviews with parties related to the above allegation, it was found that the allegation could not be corroborated, the allegation has been rendered Unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that staff are not meeting the infant’s diapering needs. No deficiencies were cited as a result of this inspection.

An exit Interview was conducted, and a copy of this report was read, Appeal Rights, and a Notice of Site Visit, were discussed and left with director Sandra Lona Aguirre at the facility. Failure to maintain the posting of the Notice of Site visit will result in a $100 Civil Penalty.
