On 8/15/2023 at 9:33am, Licensing Program Analysts (LPAs) Andrea Pittman and Donna Maddox conducted an unannounced case management visit at the facility and was met by Director Vonda Perry & Assistant Director Africa Jones who permitted entry to the facility. LPA toured the facility with the Director according to the facility sketch. Upon arrival, LPA observed 4 children with 1 staff member providing care and supervision.

On 8/2/2023, around 1:40pm, LPA toured the facility with the Director Vonda Perry. The LPA and Director walked into the infant room and observed one staff member providing care and supervision to six infants and one preschooler in the infant room. The infants were walking around the infant room. There was one child from the preschool room who was sleeping in the infant room. The preschooler was sleeping in the infant room for some time on the floor. The Director was asked why there was 7 children in the room and the Director stated she did not know why. The Director was advised that the licensed capacity of the room is only 6 infants and that there needed to be an aide or another teacher in the room with the Infant Room Teacher. The Director disclosed that they were giving lunches and that the staff would be returning. The Director spoke with the Assistant Director and later returned and admitted that there was one preschooler from the preschool room that had joined the infant room to sleep. The preschooler was sleeping in the infant room for some time on the floor. The Preschooler was then led out of the infant room to rejoin the Preschooler classroom. The Director and Assistant Director admitted that at the end of the day, the infant, preschool, and school age classrooms are commingled and combined in the infant play room where the children are placed to watch movies as the teachers are no longer present. This is evidenced by the time sheets showing the latest time a teacher is present is 6pm. Additionally, Child 3 disclosed that the children commingle and watch movies at the end of the day in the infant room.

The Directors admitted that they commingle the age groups in the infant room with the Director’s office and infant play room doors’ open where they watch the children until the parents pick them up from the center. On 8/2/2023, LPA Pittman observed that the facility was commingling when they had 6 infants and one preschooler sleeping on the floor in the infant room to the one teacher providing care and supervision while the facility was providing staff assisting in the infant classroom a lunch break. This is a Type A citation, see the LIC 809D for the details. Additionally, the facility was over capacity with 7 children in the infant room. This is a Type A citation, see the LIC 809D for the details. Civil penalties will be assessed for the violation of the fire clearance.

As a Type A deficiency has been cited, a copy of the citation and licensing report must be posted for 30 days.

The same report must be provided to Parents/Guardians and the Acknowledgment of Receipt of Licensing Reports LIC 9224 must be signed by Parents/Guardians of all enrolled children and any newly enrolled children in the next 12 months following the citation. If these requirements are not met, civil penalties per violation will be assessed.


An exit interview was conducted, a copy of this report read and provided to the Director, and the Notice of Site Visit and Appeal Rights was explained and given to the Director.
