Licensing Program Analysts (LPA's) Maddox and Pittman met with Applicant, George Smith and Director, Roshonda Dorris for the purpose of conducting a Pre-Licensing inspection for a preschool component (children ages 2 yrs through 5 yrs) with a Toddler Option (ages 18 mos to 36 mos). The hours of operation will be Monday through Friday from 6:00 am to 6:00 pm.

INDOOR ACTIVITY SPACE:

· The childcare center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees, and visitors


· Floors of all rooms have a surface that is safe and clean (wood flooring)
· A comfortable temperature for children shall always be maintained.
· Furniture and equipment are maintained in good condition, free of sharp, loose, or pointed parts. There are a variety of age-appropriate equipment, toys, and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Tables and chairs were present to meet the needs of the children.
· Drinking water is readily available via filtered water cooler
· Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children (stored in high cabinets in the kitchen/office area in a locked cabinet.)
· There are fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children:

·The isolation is in the office for any child who becomes ill during the day.
· LPA observed operable carbon monoxide detectors and smoke detectors (hard wired). Fire extinguishers are located throughout the facility and fully charged.
· The center has a working telephone
· Sign-in and out procedure will be completed manually by the representative.
· The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.

OUTDOOR

The play yard is still under construction. LPA's observed center block fencing that was at least 4ft high (possibly 5 ft). LPA's reiterated the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls and ensure the area is free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. In addition, there was no outside shade available.

· There were no bodies of water on the premises


RESTROOMS

There is 1 bathroom that has 1 toilet, 1 sink, and 1 urinal. Applicant states he will replace the urinal with a toilet and purchase another sink to cover the ratio's. There is a separate staff bathroom that will also be designated as the isolation bathroom.

SUPERVISION:

Applicant/Licensee shall ensure no child is left without the supervision of a teacher at any time, Supervision shall include visual observation.

TRANSPORTATION: Center will not provide transportation.

CHILDREN’S RECORDS:

LPA explained all children's records shall be available to the Department to inspect, audit, and copy upon demand during normal business hours.


NAPPING

· Cots will be used for napping (Applicant states cots are on order).


· Each cot/mat shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child.
· LPA informed applicant Cots or mats should be wiped with a detergent/disinfectant weekly or when soiled or wet.
· Individual bedding/linen will be stored in cubbies.

HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children (Applicant states medications will be stored in an office in a locked box and locked cabinet).

A refrigerator shall be used to store any medication that requires refrigeration.

Applicant has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

FOOD SERVICES

Center will provide breakfast, lunch, and snacks. The menu must be posted for parents’ review.

· Pesticides and other similar toxic substances were not stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. Soaps, detergents, cleaning compounds or similar substances were stored in areas separate from food supplies.


· Food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.
· Trashcans, including moveable bins, shall have a tight-fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.

POSTINGS:

Applicant was informed of the required forms that shall be posted after licensure including the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5

Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available.

Applicant is reminded of the requirement to report any unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624.

Measurements taken were as follows:
PS Classroom measured 579 which is enough space to accommodate the requested capacity of 15.

Toddler Room measured 240 which isn't enough space to accommodate the requested capacity of 15.

Bathroom:
There's 1 bathroom available with 1 toilet, 1 urinal, and 1 sink.

Outside play space measured 1,778 which enough space for each component to play outside:

Fire Clearance has been received for the requested capacity of 30 children, however with measurements taken today, center can accommodate a capacity of 24 children (17 PS and 7 Toddlers).

Needed Prior to Licensure: Applicant states he's awaiting outdoor equipment to be delivered and adequate cushioning material underneath play structure; also needs to add an additional toilet and 1 sink. Applicant will send in pictures when the play yard is set up and when the toilet and sink have been installed. LPA's will verify Director has the required immunization's. Need at least 1 potty chair for Toddler's who will be potty training and a diaper genie is needed.
