Licensing Program Analyst (LPA) Maddox conducted an announced visit today and met with Applicants, Jessica Fonseca and her husband Jaime Fonseca today for the purpose of conducting a Pre-Licensing inspection for an Infant component for children ages 6 weeks to 2 yrs. The hours of operation will be: Mon through Friday from 6:30 am to 7:30 pm. Applicant is requesting to be licensed for 54 preschool children, applicant has also submitted applications for an Preschool (197750155) and School Age Component (X197750157). This center is located small strip mall, the former Probation Office.

INDOOR ACTIVITY SPACE:

  • There are 2 separate rooms designated for Infants, the front area has cribs, feeding table, diaper changing table within arms reach of a sink. The back room contained age appropriate toys and play equipment.
  • The child care center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees and visitors
  • Floors of all rooms have a surface that is safe and clean (wood flooring)
  • A comfortable temperature for children shall always be maintained.
  • Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
  • Tables and chairs were present to meet the needs of the children.
  • There are fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children.

  • Drinking water is readily available drinking fountain, children will have their own individual water bottles. Applicant was informed that the drinking water will be required to be tested for Lead within 180 days.
  • Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children in a locked Janitors closet.
  • The isolation is located in the office for any child who becomes ill during the day.
  • LPA observed operable carbon monoxide detectors and smoke detectors(hard wired). Fire extinguishers are located throughout the facility and fully charged.
  • The center has a working telephone
  • Sign-in and out procedure (manual for now).
  • The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.
OUTDOOR

LPA observed a shaded rest area for Infants, the entire yard is surrounded by wrought iron fencing with a privacy mesh material in place. There are no bodies of water on the premises. The Infant yard is physically separate from space used by children in the child care center and school-age child care center components.

  • The play yard was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. The Infant yard has soft padding and grass, LPA observed age appropriate play equipment.

RESTROOMS

A bathroom is not required for Infants. There is a separate staff bathroom available which will also be designated as the isolation bathroom for any child who may become ill.

SUPERVISION:

Applicant shall ensure no child is left without the supervision of a teacher at any time, Supervision shall include visual observation.

NAPPING

A crib or portable-crib meeting United States Consumer Product Safety Commission safety standards shall be provided for each infant who is unable to climb out of a crib


  • Mattresses shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged.
  • Mattresses shall be made specifically for the size crib in which they are placed.
  • Cribs shall not limit the ability of staff to see the infant.
  • Cribs shall not limit the infant's ability to stand upright.
  • Crib mattresses shall be covered with vinyl or similar moisture-resistant material, wiped with a detergent/disinfectant daily and when soiled or wet.
  • Each crib, mat or cot shall be occupied by only one infant at a time.
  • Each infant's bedding shall be used for him/her only. Such bedding shall be replaced when wet or soiled, or when the crib, mat or cot is to be occupied by another infant.
  • Bedding shall be changed daily, or more often if required by Subsection (e) above.
  • Soiled bedding shall be placed in a suitable container and made inaccessible to infants until washed or picked up by a commercial laundering service.
  • Cribs shall be free from all loose articles and objects, including blankets and pillows.
  • Pacifiers shall be allowed however, there shall not be anything attached to the pacifier.
  • Bumper pads shall not be used.
  • There shall be no objects hanging above or attached to the side of the crib.

  • Cribs, mats or cots shall be arranged so as to provide a walkway and work space between the cribs, mats or cots sufficient to permit staff to reach each infant without having to step over or reach over any other infant.
  • Placement of cribs, mats or cots shall not hinder entrance or exit to and from the napping space.
  • Baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Section 1596.846.
  • Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan (LIC 9227).


HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children. A refrigerator shall be used to store any medication that requires refrigeration. Applicant has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. FOOD SERVICES

  • Weekly/Monthly menus must be posted for parents’ review
  • Pesticides and other similar toxic substances shall not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. Soaps, detergents, cleaning compounds or similar substances were stored in areas separate from food supplies.

  • Food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.
  • Trashcans, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.
  • There is a refrigerator available to store any medication that requires refrigeration.
  • POSTINGS: Applicant was informed and she is also aware of the required forms that shall be posted after licensure including the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5

Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available.

Applicant is reminded of the requirement to report and unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624B.

With regards to the outside play yard, Applicant states she will have the Infant space modified so she can accommodate at least 10 Infants, Applicant is requesting a provisional license for 6 Infants until the yard is modified. Fire Clearance has been received for the requested capacity of 20 Infants.
