Licensing Program Analyst (LPA) Maddox met with Licensee, Duana Garrett today for the purpose of conducting an announced Prelicensing/Relocation inspection. This facility is located within a strip mall and occupies Suite #'s: 121, 123, 125 127 129. This will be a combination center with School age (197750160) and Preschool (197750158) components. The Infant Toddler component consist of 2 classrooms, kitchen, bathrooms, and outside play area. The hours of operation will be: Monday through Friday from 6:00 am to 8:00 pm and Sat 7:00 am -3:00 pm . The entrance of the center is the waiting area where staff will be available to assist children and parents.

INDOOR ACTIVITY SPACE:

· The child care center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees and visitors


· Floors of all rooms have a surface that is safe and clean (wood flooring)
· A comfortable temperature for children shall always be maintained.
· Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Tables and chairs were present to meet the needs of the children.
· Drinking water is readily available from Culligan water with disposable cups

·Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children (stored in high cabinets. Stored in the back of the 2nd PS room.
· There are fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children.
  • Cribs will be lined up along the left side of the wall as you face the infant area (crib space was not included in the sq ft)
· The isolation area is located in the office for any child who becomes ill during the day.
· LPA observed operable carbon monoxide detectors and smoke detectors(hard wired).
  • Fire extinguishers are located throughout the facility and fully charged.
· The center has a working telephone
· Sign-in and out procedure manual and the Brightwheel app. -
· The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.

OUTDOOR

The outside play yard is surrounded by fencing and divided into sections to provide a physical separation for each component. The play yard was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. There is no high climbing apparatus on the Infant yard. There are no bodies of water on the premises and tarp sails cover the play yard and provide shade. Staff will bring bottles and sippy cups for outside drinking water, there is safe access to the play yard.

Licensee is reminded of the requirement to report and unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624B.

RESTROOMS

Bathrooms are not required for Infants, there is a staff bathroom located in the rear room, there is a separate lactating room and a diaper changing room is this area as well. Licensee was informed she can use 1 potty chair for every 5 toddlers that are potty training.

SUPERVISION:

Licensee shall ensure no child is left without the supervision of a teacher at any time, Supervision shall include visual observation. In addition, Licensee is aware of the requirement to check on sleeping Infants every 15 minutes and to ensure parents complete form LIC 9227 particularity Infants sleeping habits (i.e. can the infant turn over unassisted)

TRANSPORTATION:

Center will provide transportation for School age children only.

CHILDREN’S RECORDS:


Licensee is aware of the required forms for children's records. PA explained all children's records shall be available to the Department to inspect, audit, and copy upon demand during normal business hours.

HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children (kept in a locked cabinet in the office. A refrigerator is available to store any medication that requires refrigeration.

Licensee has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

NAPPING

· Cribs will be used for napping.


· Each cot/mat shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child.
· LPA informed applicant Cots or mats should be wiped with a detergent/disinfectant weekly or when soiled or wet.
· Bedding/Linen will be stored in cubbies.
  • A crib or portable-crib meeting United States Consumer Product Safety Commission safety standards shall be provided for each infant who is unable to climb out of a crib. Refer to Section 101439.1 - Infant Care Center Sleeping Equipment

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. FOOD SERVICES
· Pesticides and other similar toxic substances were not stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. Soaps, detergents, cleaning compounds or similar substances were stored in areas separate from food supplies
· Food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.
· Trashcans, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.

There is a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available.

Fire Clearance has been received for the requested capacity of 35 Infants/Toddlers. License will be approved for 20 Infants and 15 Toddlers.

The following was discussed with the licensee:
Smoking is prohibited on the premises of a child care center; Infant walkers, baby jumpers, exersaucers, baby rockers and any other item that falls into that category are prohibited; Earthquake safety and necessity of drills are to be conducted every 6 months and logged; Required forms for children’s files, facility files, staff files, and posting requirements; Fingerprint clearances and the transfer process.

The licensee was reminded it is his/her responsibility to visit the departments website to obtain licensing forms, Quarterly Updates, and Provider Information Notices (PINs): www.ccld.ca.gov
The above facility was found to be operating in substantial compliance.

Needed Prior to Licensure: Licensee will send pictures of the Infant Room set up including cribs before operating.

An exit Interview was conducted, a copy of this Report and a Notice of Site Visit was provided to site supervisor.
