32 | Allegation 1: the first allegation states that Staff leave day care children unattended. During the investigation, it was alleged that a child left the facility with the wrong person; however, this allegation is referencing an incident that occurred earlier in the year and has been addressed during a prior visit. Additionally, the allegation was made that the facility left a child unattended in a classroom. After conducting an investigation, there has been no supporting evidence of children being left unattended for the current allegation. After reviewing all the relevant information obtained, there is not a preponderance of the evidence to support the allegation.
Allegation 2: the second allegation states Staff do not ensure day care children are provided a safe environment while in care. During the investigation, it was alleged that child care children are not in a safe environment. After conducting observations, record reviews, and interviews, there is no supporting evidence for the allegation. After reviewing all the relevant information obtained, there is not a preponderance of the evidence to support the allegation.
After conducting an investigation of observations, record reviews, and interviews, it was determined that there was insufficient evidence that the facility’s staff left a day care child unattended and that the staff do not ensure children are provided a safe environment while in care. The allegations could not be corroborated with the evidence found during the investigation. Therefore, the allegations have been found unsubstantiated. Although, the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the facility operated in violation of policy in this circumstance.
An exit interview was conducted, and a copy of this report was provided to the Lead Teacher Jasmine Ruiz along with the Notice of Site Visit and Appeal Rights.
