On October 17, 2024, Licensing Program Analyst (LPA) Annelise Villa met with Site Supervisor Joyce Hart, who guided LPA on a tour of the facility. The purpose of this visit was to conduct a follow up Case Management - Incident inspection for an Unusual Incident that was received by the Department on 10/8/2024. The Unusual Incident was self reported within the time frame specified by regulations. Upon arrival, LPA observed 45 preschool children, with 10 staff present, and the site supervisor.

Description of incident: On 10/8/2024, during a classroom transition from outside back to the classroom, Staff #1 and Staff #2 were positioning to supervise children when Child #1 ran away from the staff. While Staff #1 was walking toward Child #1, Child #1 started running in the opposite direction. Child #1 ran down the facility's hallway through a single action door and into the facility lobby. The child then ran through the lobby and out the front entrance single action door and was stopped by Staff #1 approximately 5 feet outside the facility.

After interviews conducted with staff, it was revealed Staff #1 observed the incident occur on 10/8/2024, however Staff #1 was not able to catch the child before he ran out of the facility. Per Director, Staff have invested door alarms to prevent children from exiting without supervision. Staff are going training in care and supervision to prevent similar situations in the future.

No deficiencies are being cited at this time.

An exit interview was conducted, and a copy of this report was provided to the licensee, along with her Notice of Site Visit.
