25 | On Tuesday, Licensing Program Analysts (LPAs) Mayra Rivera and Sarah Rajapakse conducted a Case Management inspection to follow up in regards self-reporting incidents that occurred on May 6, 2026, and May 19, 2026.
On May 7, 2026, Palmdale Regional Office received an unusual incident report stating a staff left a child in the playground alone during transition and on May 19, 2026, a food item given to a child who has a severe allergy. During this visit, LPAs inspected the playground, checked the allergy list, medication and interviewed staff.
Based on LPAs playground inspection, the playground is fully fenced, the gates closed and no access to public approach. The weather during incident was in the 70's and child has no medical concern. The child did not sustained injuries. The staff has been terminated, and plan of action has been put in place. Transition policy and implement daily chart for transition name to face.
LPA Rajapakse observed the medication Hydroxyzine and expires 8/14/2026, Epinephrine Injection expires 9/30/26 and LPAs observed the allergy list and observed the child listed on the list with the food allergy eggs. Per interview, the child sustained a rash around the lips and medication was administered. Parent picked up child and took child to the hospital for observation.
LPAs observed present staff fingerprinted cleared and facility to be within ratio. LPAs observed a total of 21 preschoolers, 5 infants and 7 toddlers with staff #1, staff #2, staff #3, staff #4, staff #5, staff #6, staff #7, staff # 8, and staff #9 present providing care and supervision.
