32 | Based on LPA Rivera observations and interview, the preponderance of evidence standard has been met, therefore the above allegations are found to be Substantiated. California Code of Regulations, Title 22.
One type A deficiency is being issued today for regulation 101429(a)(1)- (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. Under no circumstances shall ANY infant be left unattended.
One tyoe B deficiency is being issued today for regulation 101161(a)-(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license.

Upon receipt of this report, the licensee shall post any licensing report documenting a type “A” citation. This must remain posted for 30 days during hours of operation. In addition to posting this report, the licensee will also provide copies to the parents of the children in care for up to one year.
A copy of the Parent Notification Requirements was provided to the licensee, along with a copy of the LIC 9224 - Acknowledgement of Receipt of Licensing Reports.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with director Luis Ledesma. Director was provided a copy of the appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms. |
