On Wednesday, April 1, 2026, Licensing Program Analyst (LPA), Mayra Rivera conducted an unannounced complaint inspection in regards the above allegations. LPA met with Director Luis Ledesma who granted access and guided LPA on a tour of the facility.

LPA entered the infant classroom and observed the crib be free from blankets, pillows, bumper pads, hanging items and no attached pacifier. LPA observed the crib to have a fitted mattress with a fitted cover sheet. LPA did not observed hazards.

LPA Rivera observed current staff files and observed present staff to be qualified. LPA documented on LIC 859.

This report was amended on 5/7/26 to correct LIC 9099-D on page LIC 9099-A. Facility representative signed electronic.

On Wednesday, April 1, 2026, Licensing Program Analyst (LPA), Mayra Rivera conducted an unannounced complaint inspection in regards the above allegations. LPA met with Director Luis Ledesma who granted access and guided LPA on a tour of the facility.

At approximately 11:20 am (annual inspection), LPA entered infant classroom and observed an infant sleeping in the crib and no staff present providing care and supervison. Director stated no infant teacher present today, and director has been in and out of the infant classroom. LPA informed each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times.

At approximately 11:50 am (annual inspection), LPA entered the preschool classroom and observed 5 preschoolers and 1 school-age child present with staff #2 and staff #3 present. Per director, the school-age child was a last-minute care for the day and facility does not have a school-age teacher. LPA informed director preschool and school-age children are not allowed to commingle.
This report was amended on 5/7/26 to add the plan of correction to the Type B column. Facility representative signed electronic the LIC 9099 and LIC 9099-D.

Based on LPA Rivera observations and interview, the preponderance of evidence standard has been met, therefore the above allegations are found to be Substantiated. California Code of Regulations, Title 22.

One type A deficiency is being issued today for regulation 101429(a)(1)- (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. Under no circumstances shall ANY infant be left unattended.

One tyoe B deficiency is being issued today for regulation 101161(a)-(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license.

Please see attached LIC 809-D for citations.

Upon receipt of this report, the licensee shall post any licensing report documenting a type “A” citation. This must remain posted for 30 days during hours of operation. In addition to posting this report, the licensee will also provide copies to the parents of the children in care for up to one year.



A copy of the Parent Notification Requirements was provided to the licensee, along with a copy of the LIC 9224 - Acknowledgement of Receipt of Licensing Reports.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with director Luis Ledesma. Director was provided a copy of the appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.

LPA observations and record review did not support nor confirm the above allegations. This agency has investigated the complaint alleging Staff did not ensure that the infant crib was free from all loose objects and Staff do not have the required education units or qualification At this time, it is determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. No deficiency given at this time.
