| Client Rooms 1 - 4: LPA Ramirez observed all client bedrooms to contain the required linens, furnishings, and lighting. Bedroom#1 is currently unoccupied. LPA Ramirez observed an operational manual mechanical lift in bedroom#1.
Bathroom 1-2: Water temperature in two (2) client bathrooms were within 105- 120 degrees F. Bathrooms were observed to be clean and well stocked. Grab bars were observed in bathrooms.
Backyard: No large bodies of water were observed.
Carbon Monoxide Detectors/Fire Alarm/Fire Extinguisher & Emergency Disaster Plan: LPA observed carbon monoxide and smoke detectors in hallways. Smoke detectors were observed to be operable during visit. Fully charged fire extinguishers were observed throughout the facility.
Centrally Stored Medications: LPA Ramirez observed centrally stored medications to be inaccessible to clients in care. Medications are centrally stored in a kitchen cabinet.
Backyard: Well-maintained with plenty of shade and free of hazards.
Emergency Drills: Proof of last documented emergency drills were conducted on 01/05/2026.
Staff Personnel Files: Staff files are maintained at the facility. LPA Ramirez observed an Administrator’s Certificate for Josephine Sapalaran (7001651735) expires 04/01/2027. Alternate Administrator Omar Sapalaran (70001651735), certificate expires 06/02/2027.
Client Files: Three (3) client files were reviewed. Admission Agreement, Physician's Report (including T.B and Ambulatory Status), Consent for Medical Treatment, Preplacement Appraisal Information, Resident Pre-Appraisal, Care Plan/Appraisal/Needs and Services Plan, Resident Rights, and Individual Program Plan were observed. LPA did not observe an age exception for client#2. LPA Ramirez will issue one (1) Technical Violation based on record review.
Infection Control Plan: LPA Ramirez reviewed current infection control plan.
Exit interview was conducted. No violations were observed. A copy of this report, and LIC 9120 was provided.
