32 | Investigation revealed the following:
Allegation: Staff does not prevent clients from having illegal drugs in the facility.
It is being alleged that facility staff do not prohibit clients from bringing in and using illegal drugs at the facility. On 02/11/2026 LPA Watson interviewed Administrator Ahmed Saafir (A1). During the interview conducted on 02/12/2026, A1 said that they observe and request that clients don’t bring drugs of any kind into the facility and that they inspect their rooms, and drawers on a regular basis to ensure no drugs enter the facility. A1 also said that they do not know of any client using drugs in the facility. On 02/12/2026 LPA Watson conducted interviews with Staff#1- #3 (S1-S3). Out of those interviewed 3 out of 3 staff members denied the allegation. On 02/12/2026 LPA Watson conducted interviews with Clients#1- #4 (C1-C4). Out of those interviewed 3 out of 4 denied the allegation. LPA Watson obtained and reviewed an incident report for R1 dated 10/24/25 and it did not show any incident of drugs being used at the facility. Based on information gathered and records reviewed there is insufficient evidence to support the stated allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. No deficiencies were cited.
Allegation: Staff does not allow clients to have private visits with clients’ visitors.
It is being alleged that facility staff prevent R1 from having private visits with his relatives in his bedroom.
On 02/11/2026 LPA Watson interviewed Administrator Ahmed Saafir (A1). On 02/11/2026 LPA Watson interviewed Administrator Ahmed Saafir (A1). During the interview conducted on 02/12/2026, A1 said non-residents aren’t allowed to go into the clients’ rooms inside the facility, because they are shared rooms and it would be a privacy and liability issue. On 02/12/2026 LPA Watson conducted interviews with Staff#1- #3 (S1-S3). Out of those interviewed 3 out of 3 staff members denied the allegation. On 02/12/2026 LPA Watson conducted interviews with Clients#1- #4 (C1-C4). Out of those interviewed 3 out of 4 denied the allegation. LPA Watson conducted interviews with A1, S1, S2, S3, C1, C2, C3, and C4 and all staff and clients interviewed stated that they could have outside visits in the picnic area. Based on information gathered and records reviewed there is insufficient evidence to support the stated allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. No deficiencies were cited.
