Per Licensee, currently home has no poisons. LPA reminded Licensee when poisons are kept in the home must be locked not only inaccessible to children in care. Detergents, cleaning compounds and other hazardous items are made inaccessible with baby latch in place; observed under kitchen sink. LPA observed fire extinguisher was serviced on March 24, 2025. Knives are located in high inaccessible area on top of refrigerator. Per Licensee, food is provided to children in care.
The home has a working telephone service. Per Licensee, no children on medication. Per licensee, no firearms are kept on premises.
The outdoor play area in the backyard is fenced. LPAs observed no swimming pool or bodies of water.
Posted documentation's observed and located in the living room: Facility License, PUB Parent Rights notification and Earthquake Preparedness form.
LPA reviewed children and staff records. Two infants files were reviewed both infants present Infant #1 and #2 had inconsistent 15 min sleep log records that did not show time indication -15 min checks, deficiency cited. Infants had all other documentation's in place.
LPAs reviewed 6 children’s file total. Per children’s file review, LPA observed the following documentation's in place for Child #1 - Child #4 records: Emergency and Identification Information form, Consent for Medical Treatment, Immunization records, and Affidavit Regarding Liability Insurance. LPA printed out Addition Children in Care form- LIC9150 to add to files.
Staff Records was reviewed: Licensee Mandated Reporter expired 6/2/25, Staff Assistant #1 (S1) Mandated Reporter Training was not provided, Staff #2 (S2) mandated reporter training expired 6/9/2024; picture of certificate , deficiency cited. LPA reviewed Pediatric First Aid and CPR , Licensee, S1 and S2 expired 2/27/25, picture taken of certificate, deficiency cited.
Disaster and Fire Drill was last conducted on May 18,2025. Staff Interview was conducted with Licensee.
