Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced annual required inspection at the above facility on 06/28/2023 at 09:40 AM. LPA met with Lydia Hunter, Licensee who guided analysts on a tour of the facility. There was 1 infant and 5 children present during today's inspection. Facility capacity is in compliance for a large Family Child Care Home. Hours of operation are Mon-Fri 8:00 AM - 5:00PM.

Care is provided in the living room. The kitchen and dining room are also accessible. The second floor is off limits to children in care. There are two bedrooms and one bathroom on the second floor. The two bedrooms in the hallway adjacent to the kitchen are off limits while the guest bathroom is accessible. There were no fireplaces, wall heaters, pets, or detached sheds observed in the home. The backyard is off limits while the front yard is used for outdoor activity space. Children have no access to the garage. All rooms that are off-limits need to be made inaccessible during operating hours. LPA observed the home to be kept clean and orderly, with heating and ventilation for safety and comfort. Staircase was observed to be properly barricaded. Capacity and ratio was observed to be in compliance. Licensee complied with inspection authority.

Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:

LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 5/22/23.

Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully and was service on 02/02/23. The home maintains telephone service via cell phone and landline. There are toys and other age appropriate material available for children. LPA observed that detergents, cleaning compounds and medication are inaccessible to children. Licensee understands that all poisons must be locked. Isolation area for sick children waiting to be picked up is in the hallway, away from the other children. Per Licensee there are no firearms or weapons stored in the home.

Licensee cares for 1 infant. LPA observed 1 play yard visible in the living room. Napping equipment does not block entrances or exits. Infant mattresses were observed to be firm with tightly fitted sheets. LPA did not observe loose object, bumpers, objects hanging, or objects attached to the play yards. LPA reviewed with Licensee the new Safe sleep regulations and the 15 minute sleep check documentation for infants 0-24 months. LPA provided PIN 20-24, LIC 9227, and a 15 minute log. Licensee does not provide any overnight care at this time.

Currently, children are using the front yard for outdoor play. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that could be hazardous to children in care. There is a pool in the backyard. Pool is an off-limit area and is gated. The gate is iron and is over 5 feet.



Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights. Staff records were reviewed for approved Pediatric First Aid and CPR certification expires on 03/19/2024, LIC-501: Personnel Record, LIC 508- Criminal Record Statement, LIC 9052- Employee Rights.
During file review at 10:45am, Licensee did not have proof of immunization against measles and pertussis. At 10:56am, Licensee called her primary doctor to check if she has had TDAP and MMR. Per person on the phone, Licensee has had her TDap in the pass and in 2014. Licensee did not have a MMR but requested a same day appointment but the soonest appointment was 07/03/23. Licensee declined influenza during inspection, TB clearance or risk assessment, LIC 9108- Statement Acknowledging.
Licensee has not had a visit since 2018, the year the Mandated Reporter went into place. Per Licensee was unaware of the training. LPA gave Licensee information on how to obtain training. LPA asked Licensee if she can obtain training within the next 30 days.

During inspection all children were observed to be treated with dignity and respect, they were observed to be receiving safe, healthful and comfortable accommodations, furnishings and equipment, and free from corporal and/or unusual punishment.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Consult was given regarding the following:

- Vacation: LPA collected the list of days off for the year of 2023

- Decreasing License to a Small FCCH

- IMS

- LPA collected email to send Licensee the Ratio chart for a Small FCCH and an application

At 11:52am, Licensee conducted a fire drill and the children were all prepared and knew what to do and all lined up at the door.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care- licensing/public -information-and-resources/safe-sleep as an additional resource.

LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Based on the LPA observations and records review no deficiencies will be cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Licensee L. Hunter. Appeal Rights were given and explained.
