LPA White reviewed diapering logs going back to January for each class, and conducted observations of the classrooms on 1/14, 1/30 ,and 3/4. During those observations LPA White observed staff routinely taking day care children to the bathroom, and checking the children before returning to the classroom. Each staff member documented, or shared the information with the teacher in the classroom to ensure accurate documentation for each child. Site Supervisor stated that when facility is short staffed, teachers are instructed to take all the children to the bathroom together to ensure every child's needs are met.
