Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 6/30/23 and was reported to the Department on 6/30/23. A COVID risk assessment was conducted upon entry and LPA wore appropriate personal protective equipment. LPA met with Director, Cindy O'Neill to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. Census was taken. There were 33 children present during the tour of the facility. The facility was observed to be operating within the license capacity limitations.

On June 30, 2023, an unusual incident report was made to the Department regarding an incident that involved a child who sustained an injury that required medical attention. The facility reported this incident to the Department within the required 24 hours. Based on information obtained during interviews conducted with Director and parent, LPA Babcock determined that during an end of the school year event, the child lost his footing during outdoor play and fell onto the cement coping surrounding the play structure area, hit their right arm on the cement coping, and an injury was sustained wherein the child fractured their right forearm. The injured child was at the facility on the date of LPA visit, but did not wish to be interviewed. At 9:50 a.m. during this inspection, LPA inspected the outdoor area and did not observe any hazards near or on the area where incident took place. Based on the interview with the Director, who was the only staff that witnessed the fall, and the parent, who was present at the time of the incident with her husband, and also observed the incident, there was adequate supervision at the time of the incident. present, they could not prevent the injury. As the parents were on site at the time of the incident, and observed the incident, the child was taken to the hospital by parents and per the medical report from the hospital which LPA received from the facility, the forearm was fractured and a cast was applied to the right arm. Child has returned to day care.

Based on information obtained during this investigation, no additional follow up is necessary regarding the incident reported. The facility followed all proper procedures; child’s parent was notified, incident report was sent in properly and timely, and all medical needs were met.

No deficiencies are being cited at this time.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

As Director left for a meeting at 11:30 am, exit interview conducted and report was reviewed with Center Coordinator, Lupe Roque.
