Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced case management inspection for an Action Level Exceedance (ALE) detected in a water fixture in the facility. Upon Arrival at 1:40pm, LPA met with Lead Teacher Wenli Li, who guided LPA on a tour of the facility. There were children present during the time of the inspection. This facility is a part of the Pasadena Unified School District (PUSD), on Field Elementary School's campus.

Census was taken. LPA observed Rooms H25 and C21, there were 17 children with 5 staff members.

LPA reviewed new Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, which requires the testing of water for lead in Child Care Centers (CCCs) with facility director during the inspection. Per AB 2370, all CCCs that are located in buildings constructed before January 1, 2010, must have their water tested and post the results by January 1, 2023, and every 5 years after the date of the first testing.

Facility provided facility sketch and required forms LIC 9276, LIC999 and LIC9275 to LPA during today’s inspection.

On 01/10/2024, the Department received notification from the State Water Resources Control Board (SWRCB), Division of Drinking Water (DDW). The SWRCB report indicated the facility was inspected and samples were collected on 01/05/2024. Faucets and drinking fountain reported with 5.5 ppb or greater lead exceedance levels were as follows:

· C - 1st Floor C104 Workroom - Faucet - Test Result 14.9ppb

The water outlet afftected is in the "work room" for classroom C22. C22 is a Transitional Kindergarten class that is not licensed through the Department of Social Services. LPA contacted PUSD Coordinator Patricia Guzman regarding the repeated testing of a water outlet in a non-licensed room/area. Coordinator contacted the Field Elementary Principal Charlene Tucker, who spoke with LPA regarding the issue. LPA explained that the water samplers have been instructed to test the incorrect water outlets, and the results are showing exceedance for non-licensed rooms/areas. REPORT CONTINUES PAGE 1 of 2

Principal had a member of maintenance present during the explanation of the incorrect outlets being tested. They clarified that another member of plumbing typically is the one who opens the classes for the Samplers in the morning for the testing.

Classrooms C21 and H25 are the only classrooms that are licensed by the Department, and the children do not have any access to C22 or the C104 workroom attached to it. Lead Teacher states that children bring their own water bottles and that there are plastic water bottles available for children who run out of water or who forget their water bottle. LPA observed the supply of water bottles. At this time there is not a lead exceedance for the licensed areas on campus. LPA instructed the Principal and Coordinator that they should not continue to test the non-licensed rooms of the school campus.

Grant funding will be available for testing and remediation of lead to the Child Care Centers that qualify. To make a determination of eligibility, refer to PIN 21-04-CCP. For Lead Testing and Prevention Information, including additional resources please visit

The facility is not being cited as none of the faucets with lead exceedance are being used for consumption or food preparation.

Notice of Site Visit was provided and must remain post for 30 consecutive days. A copy of PIN 21-21-CCP and appeal rights were provided to facility today.

An exit interview conducted with Lead Teacher Wenli Li, at 3pm, and a copy of this report was provided.
