25 | Licensing Program Analysts (LPAs) Jeanette Estrada and Jonnisha Culbert conducted an unannounced case management inspection to follow up on an incident that was reported to the Department on 5/14/24. Upon arrival, LPAs met with Head Teacher, Martha Villafana. LPAs informed her of the reason for the visit. There were no children present upon LPAs' arrival.
It was reported to the Department on 5/14/24 that three children were diagnosed with Hand, Foot and Mouth Disease (HFM). During the today's visit, LPAs were informed that the children listed on the report are enrolled in the Early Head Start classroom under license number 198019033. The Early Head Start classroom is on the same premises as this facility. LPA advised Head Teacher to have the HFM diagnoses reported to the Department under the correct license number. Per Staff, there have been three cases diagnoses since 5/14/24 for this facility. LPA advised for these three cases to be reported to the Department.
Per Staff, the facility has increased sanitizing and cleaning throughout the classrooms and parents have been notified. Cases have been reported to Long Beach Health Department.
No deficiencies cited.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
