32 | C1 was taken to the Emergency Room (ER) where it was determined that C1 fractured their clavicle. Licensee followed up with C1s mother to inquire about the child’s well-being. Per text message, mom sent pictures of C1 with a sling and a doctor’s note that stated C1 can return on 11/20/24. Per licensee, C1 is no longer attending the facility. LPA took pictures of the activity room and child sized chair. Licensee and co-licensee have since removed all children’s chairs from the activity room to prevent future incidents or accidents. The incident happened so fast that it couldn't have been prevented.
During this inspection, LPA interviewed licensee and co-licensee. LPA reviewed child 1 (C1) file and took pictures of the activity room, chair, and ouch report.
At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative, a civil penalty of $100 can be assessed.
