Licensing Program Analyst (LPA) Austin Estrada conducted an unannounced case management visit to follow up on Child Abuse Central Index (CACI) possible match for Luis Lopez in association with the above facility. LPA met with Director Jessy Ogbuefi, who guided LPA on a tour of the facility. LPA observed 13 children and three staff at the time of inspection.

The purpose of the visit was to determine if Luis Lopez currently works at the facility. Director Jessy Ogbuefi stated that Luis Lopez applied for the position of housekeeper in February 2022. Per Director, Luis Lopez’s fingerprints did not clear which is why he never worked for the facility.

LPA spoke to Administrator Lisa Square who stated that the previous Administrator, Barbara Holmes, tried to disassociate Luis Lopez from the facility roster in November and December 2022, but were unable to do so. Director Jessy Ogbuefi provided LPA with a printout list of associations and a written declaration that states Luis Lopez never worked for the facility and to have Luis Lopez disassociated from the facility.

LPA did not observe Luis Lopez during today's visit. LPA was provided with a current staff roster/schedule.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. This report along with a copy of the appeal rights was provided. Exit interview was conducted with Director Jessy Ogbuefi.
