13 | Licensing Program Analyst (LPA), T. Tran arrived at the above licensed facility to conduct an unannounced subsequent complaint inspection for the purpose of concluding the investigation of the above allegations. Upon arrival, LPA met with Cristina Hinojosa, Director.
During today's visit, facility representative provided LPA the tour of the facility. LPA observed the outdoor play area is clean and orderly. LPA conducted interviews with children. Based on the evidence obtained during the course of the investigation through interviews, record reviews, and observation, the evidence does not support, nor disprove the above allegations of the facility outdoor play structure and materials were dirty or had urine odor occurred at the facility. Therefore, the allegations have been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
A notice of site visit was given and must remain posted for 30 days.
