13 | Licensing Program Analyst (LPA), T. Tran conducted an unannounced visit for the purpose of delivering the findings for the above complaint allegation. Upon arrival, LPA met with Site Director, Aurora Ortiz and Program Manager, Cristina Hinojosa.
Based upon the evidence obtained through record reviews and interviews with staff, children, and others. The evidence does not support, nor disprove neglected care/supervision and personal rights allegations happened at school. During the interview conducted on 7/22/25, parent of C1 (See LIC811) did not have any concerns with the staff that accidentally smashed C1’s hand when closing the door. In addition, records revealed multiple minor incidents occurred to C1 did not cause by an ensure safe environment. Staff indicated to prevent C1 from getting bitten by other toddlers in the class, the facility had a staff to shadow C1 with the ratio of 3:12. Therefore, the allegation has been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the facility representative, Aurora Ortiz.
