13 | Licensing Program Analyst (LPA), T. Tran conducted an unannounced visit for the purpose of delivering the findings for the above complaint allegation. Upon arrival, LPA met with Site Director, Aurora Ortiz and Program Director, Cristina Hinojosa.
Based upon the evidence obtained through record review and interviews with staff, children, and others. On 12/8/25, S1 (See LIC811) shared, during naptime staff went to place an item in the children’s restroom not realizing C1 was behind then accidentally closed the door on C1 and sustained a red/puple bump on the forehead. For the allegation of staff does not ensure C1 noses clean because C1 dislike to be touch and wash hands with soap therefore, often staff used wet wipes to clean C1 hands and face. The above evidence does not support, nor disprove the child’s personal rights allegation have been violated while in care.
Therefore, the allegation has been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the facility representative, Aurora Ortiz.
