At 9:45am (LPA) Licensing Program Analyst Stephanie Li conducted an unannounced annual inspection. Facility has requested to reopen from inactive. Annual visit was conducted on 11/03/2023. Facility was not ready to operate and had corrections to fix. Upon arrival LPA met with Office Adminstrator Firly Septiana. Administrator called Director Haleema Shaikley and Director was on her way. LPA entered the classroom and noticed consecutive beeping from the carbon monoxide detector. LPA observed reoccuring beeping every 30 seconds. LPA informed administrator and contacted LPM Gabelman. LPA called 911. Fire department came out and checked the area for carbon monoxide and did not detect anything. Fireman checked the carbon monoxide machine and detector was expired in 2016. Fire men informed us that carbon monoxide machines have a shelf life and will not operate properly once they have reached the shelf life. Fire men stated that 5 beeps signals end of machine life and that 4 really loud beeps mean carbon monoxide detected.

Director showed up while fire men were leaving. 0 preschool children were present as facility has not re-opened yet. There are 3 children enrolled waiting to start. K-12th Private school was operating and classrooms were full of students. Hours of operation are Monday-Friday 8:30am-3:00pm.

All areas identified on the Facility Sketch were inspected. The facility consists of 1 classroom and previous outdoor playspace is closed due to construction. Final construction is pending review of grant funding. Facility is requesting a play ground waiver to use the school age playground until their outdoor play space is finalize. The Licensee is within the conditions, limitations, and capacity specified on the license.

Licensing staff observed disinfectants, cleaning solutions, medication and other items that are dangerous to children were not accessible to children. Classroom furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All indoor floor areas need to be cleaned. LPA observed dead ant colonies near the back door leading outside and several debris scattered across the room. Per Director, they are still in the process of setting up the classroom. Per Director, the teacher was here over the weekend rearranging the new furniture. LPA observed new furniture and room has been set up with age appropriate toys and equipment. page 1 of 5

Napping equipment (mats) and bedding were inspected for good condition, appropriate storage, and cleanliness. Children take their blankets home to wash weekly. Storage for children's belongings were inspected. All toilets and washing facilities were inspected for safe and sanitary operating conditions. LPA observed the right toilet to leak and spout water onto the floor at the back of the toilet when flushing. Deficiency is being issued for leaking toilet pipe. First Aid supplies are available. Drinking water was readily available indoors via tested water fountain. The schools office is used as an isolation area when a child is sick, there is a chair available for an ill child to rest and the restroom in the hallway will be used if needed.

Licensing staff observed all required forms/publications to be posted by the front entrance. Snack menus were reviewed to ensure that they are being posted one week in advance where it is visible by the child's authorized representative. Facility provides am snack. Children bring lunch from home. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was inspected to be free of flies, other insects, and rodents. LPA observed ant colonies dead and alive inside the base of the door leading outside.

Licensing staff observed the required 2A10BC fire extinguisher indicates last purchased 11/15/2023. Per State Fire Marshall standards, fire extinguishers shall be serviced or purchased annually. Facility does not have a functioning carbon monoxide and deficiency is being issued. Smoke detector is hard wired to fire department and meets statutory requirements. Director states there are no weapons, firearms, or bodies of water on the premises. LPA did not observe any bodies of water during visit. Per Adminstrator, the last disaster drill was conducted 8/27/ 2024 during fire department visit and the log is kept in the schools administration office.

At 11:30 am, licensing staff reviewed the outdoor play area. The preschool facility does not currently have an outdoor play yard. Facilities previous play yard is under construction and gates have been torn down to repave the flooring. Facility is requesting a waiver to temporarily use the private K-12th school age playground until grant funding is available to continue rebuilding of their outdoor play yard. Waiver letter was collected during visit and photos of route to yard and play ground has been captured.

LPA measured 3 different potential outdoor play spaces for construction to ensure that the finalized play areas could accommodate center's capacity of 14. Director stated that she would like to get the classrooms reassessed in the future to increase the capacity utilizing the 2nd classroom that was previously used as a storage room during initial licensure. LPA made outdoor measurements to include the potential outdoor play space of a capacity increase based on the 2nd classroom indoor square footage.

The playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Drinking water is readily available outdoors through water fountain.

Sign in and out sheets were available but not in use due to re-opening. Administrator was reminded to ensure that the person who signs the child in and out must use their full legal signature and records the time of the day. Licensing staff reviewed staff records documented on the LIC 859. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza. Children’s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857.

There are no children currently on medication. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

The facility has completed lead testing and is in the process of remediating lead exceedances. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.


Based on Licensing staff observations and interview, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
