Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced annual required inspection. LPA met with Licensee Susan Grace who guided analyst on a tour of the facility. There were six day-care children present when LPA arrived. Facility capacity is in compliance for a Large Family Child Care Home. Hours of operation are Mon-Fri 6:00 AM - 6:00PM.

This is a two story home which consists of 4 bedrooms and three bathrooms. Areas used by the children include the kitchen (as needed), dining area (as needed), den (napping & eat only), one bathroom, living room, and garage. Areas that are off limits to children and parents include: media room, complete second floor, all bedrooms, two bathrooms, front yard, and back yard. Food is provided by Licensee. Licensee was reminded if children bring food from home it must be labeled with the child’s name and properly stored or refrigerated. During inspection, Licensee informed LPA that the media room is no longer being used.LPA is requesting an updated facility sketch to make the media room and "off limit" area.

At 1:30pm, LPA observed the children napping in the garage. Per Licensee, children usually sleep in the den, however, due to all children being sick, they are all napping in the garage away from all other family members that live in the home. LPA did observe children to appear to be ill.

The following was observed and reviewed during this inspection:
LPA reviewed required posted documentation for Facility License and Publication (PUB) 394- Notification of Parent Rights. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 01/12/25.

At 1:40pm, LPA observed detergents, cleaning compounds accessible to children in the hallway between the garage and bathroom. Photo was taken. Licensee placed items in a high cabinet. Type B was cited.

Smoke and carbon monoxide detectors are in operable condition. Fire extinguisher indicated fully charged but has not been serviced within the year. The home maintains telephone service via cell phone and landline. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children. Stairs are screened to prevent access by children. Licensee understands that all poisons must be lock, not only inaccessible to children. Isolation area for sick children waiting to be picked up is located in the living room, away from the other children.

Per Licensee, there are no longer any firearms or weapons stored in the home. The bathroom that children use was observed to be clean and free of hazards.

Per Licensee there are no infants in care at this moment. LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/ public-information-and-resources/ safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc .gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Currently, children go to Perry Park for outdoor play, located at 21520-21590 S. Perry St., Carson 90745 LPA reminded Licensee that visual supervision is required at all time while the children are at the park. LPA observed a pond/fountain located in the front of the home, which is off limits. LPA observed the pond/fountain to be screened with a net and had no water.


Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15 minute Infant Sleep Check (0-24 months).

Staff records were reviewed for approved, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate (expires 05/09/26).

At 2:10pm during staff file review. LPA did not observed an EMSA approved CPR and 1st aid training. Per Licensee was unaware. Type B was cited.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) policy was discussed. For IMS information , see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the LICENSEE Susan Grace, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22, Division 12, Chapter 1 and Section CCR & H&S.



A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given and explained. Exit interview conducted and report was reviewed with the Licensee, S. Grace.
