On 09/11/26 at 12:30pm, Licensing Program Analyst (LPA) Diana Ortiz conducted an unannounced annual random inspection of the above facility. LPA entered facility through the main office of Sumner Danbury Elementary school. There is a side door that leads outside to the state preschool classroom 601. LPA met with Child Development Teacher on Special Assignment (TOSA), Esther Adams who guided LPA on a tour of the facility. The program operates as a State Preschool program which operates Monday-Friday, 8:00 A.M.-11:00 A.M. and 12:00 P.M.-3:00 P.M. The program is currently only operating in the AM due to low enrollment. LPA observed a census of zero (0) children and zero (0) staff present. All areas identified on the facility sketch were toured and inspected both indoors and outdoors. Per Facility representative there are currently 15 preschool children enrolled. LIC 125 Entrance Checklist provided.

The following documents were posted in a prominent, publicly accessible area at the facility: Facility License, Notification of Parents' Rights (PUB 393), Personal Rights (LIC 613A), Menu, and Earthquake Preparedness (LIC 9148). PUB 269 Child Passenger System Poster, and Daily Schedule.

The preschool program consists of one (1) classroom. This is one large classroom which is near the main office and located towards the front of the school. Next door to the State preschool there are two Special Day Classes and classrooms are located west of the State program. During classroom inspection, LPA observed two (2) restrooms which had child size toilets, and one (1) child sized sink. All toilets and washing facilities are in safe and sanitary operating conditions. LPA also observed an adult size sink, refrigerator, adult size desk, a storage room, and a workroom for staff to utilize. LPA observed that each preschool child has labeled storage for their belongings.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Each cubby is labeled with each child’s name. LPA observed age-appropriate furniture, equipment, and toys to be in good repair and free of sharp, loose, or pointed parts. Classroom floors were clean and free of hazards, and all materials and surfaces accessible to children were observed to be free of toxic hazards. The facility was operating within the conditions, limitations, and capacity specified on the license.

Sign in and out takes place in the entrance area of the classroom. The facility utilizes paper sign-in and sign-out sheets for parents to document the arrival and departure of their children. Records were reviewed to ensure that the individual signing the child in and out provides a full legal signature and accurately records the time of entry and exit. LPA reminded the Director that parents are required to use their full legal signature when signing their children in and out each day. The facility maintains sufficient hard copies of sign-in and sign-out sheets as needed.

Carbon monoxide were tested during this visit and is working and operable. Smoke detectors were not tested during todays inspection due to the detectors being wired to the emergency pull down fire alarm system. LPA observed a fully charged and serviced fire extinguisher. The fire extinguisher was last serviced on 08/05/2026, and service tag was attached. Emergency disaster drill was last conducted on 09/03/2026. LPA observed that first aid kits were maintained in the classroom. Heating, lighting, and ventilation were evaluated and found to be adequate.

LPA toured the outdoor playground area. The playground is located down the outdoor hallway approximately 80 feet away from the State program and is surrounded by a chain-link fence and brick wall. The playground is shared with the Special Day classes; however, they are on different schedules. A waiver for shared activity space on a staggered schedule is on file and was observed to be posted on the parent board. Playground equipment, including play structures, tricycles, and play toys, were observed to be in good condition and free of sharp, loose, or pointed parts. The outdoor activity space is maintained in a safe condition and observed to be free of hazards. Areas surrounding and beneath climbing structures, swings, slides, and other elevated play equipment were adequately cushioned with rubber material. There are several trees that provide adequate shade in the play yard. There is a sandbox located in the outdoor play area, per staff, it is cleaned every morning and covered at the end of the day with a tarp. LPA observed that the school has a physical therapy pool that is located across the classroom behind the main office and adapted P.E. room. which is utilized by the Special Day classes.
