Licensing Program Analyst (LPA) Alicia Mooberry conducted an unanounced Random Annual Inspection on this date. Upon arrival at 10:00am LPA met with Sheila Hall, explained the purpose of inspection and provided the inspection Entrance Checklist, LIC 126. Licensee provided tour of facility and LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access and the areas that are inaccessible. Per licensee the current hours of care provided are Monday – Friday 7:00am - 6:00pm. The licensee offers overnight care, LPA discussed with licensee that "Overnight Care" means care being provided to children anytime between the hours of 6 p.m. and 6 a.m. Care provided during the day and overnight combined shall not exceed 24 hours from the time the child entered care.

At approximately 10:45am, Shefelia Campbell, Assistant, arrived at the home with 4 children. Also present was a 16 year old assisting with paperwork. All adults present have been cleared and associated. Individuals residing in the home were discussed and noted.

This is a single story home with three (3) bedrooms and three (3) bathrooms, an attached garage (converted to daycare space).


Areas that are used by children are: Dining Room, Converted garage, backyard and bathroom in converted garage. Per licensee, she has always used the garage as daycare space. LPA advised that the Fire Marshall does not allow for garage to be used for daycare unless it has been permitted for residential use. Per licensee the children eat outside and in the dining area of home.
Off limit areas: Three (3) bedrooms living room and 2 bathrooms. The backyard is used for outdoor play as well as the nearby park.

Areas that are used by children were inspected for safety, comfort, cleanliness, telephone service, ventilation, and heating. The home has central air and heating. There is a fire place in living room. That is barricaded

Body of water on site in back yard: LPA observed a covered and locked jacuzzi in the backyard.

The jacuzzi was full of water. Children use the back yard as outdoor play area. Per licensee, the children are always supervised when they are playing in the backyard.

LPA observed that there are a number of standing heating lamps (requiring propane), an outdoor grill connected to natural gas, lawn mowers and other items that need to be made inaccessible to children in care to avoid a potential injury or harm to children in care.

LPA discussed and reviewed the documents to be posted in a prominent location and documents that need to be reviewed during inspection (LIC 126)



LPA reviewed Facility files, children's files and licensee and assistant files. The following was missing or incomplete:
  • 4 out of 4 children's files were missing required Licensing forms.
  • The children's Roster, was not available for review
  • Required training including Pediatric CPR & First Aid, mandated reporter (AB1207) is expired for Licensee and assistant
  • The Fire Extinguisher has not been serviced in the last 12 months.


LPA provided technical assistance to licensee on Physical Plant, facility administration and records.

No deficiencies were cited during this inspection, Due to time limitations, this inspection will be continued at a later date, where deficiencies will be cited.
