13 | On 07/30/2026 at 1:15PM Licensing Program Analyst (LPA) J.Duran conducted an Unannounced Complaint Inspection for the purpose of delivering findings for the above allegations. LPA announced purpose of inspection and was allowed entry to facility by Licensee Sahara Hernandez. During the investigation LPA made observations, conducted interviews, and obtained records. Information gathered from interviews did not corroborate the RPs allegation that Licensee allowed uncleared adult to reside in the home. Licensee stated that her son Antonio Madrid in fact lives in ADU with different address 1551 ½ E 76th St and is always working. Staff Interview stated that no one other than the licensee lives in the home. Children interviews stated that no one other than Licensee and assistant are always home. LPA did not observe other male adult belongings other than licensee's. This agency has investigated the complaint alleging Licensee allowed an uncleared adult to reside in the home. We have found that the complaint was unsubstantiated meaning, a finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
