25 | Licensing Program Analyst (LPA) Randy Derraco conducted a case management visit to the above mentioned facility on 08/03/23. LPA arrived at the facility at 10:35 AM and was met by Director Charlot Garza. Per Director, she was currently in a meeting and was unable to accompany LPA on tour of the center. This is a preschool program which consists of 4 classrooms. Classroom 1A and 1B are part of the full day program that operates Monday thru Friday from 7AM - 5PM. Classroom 2 and 3 are part of the half day program that operates Monday thru Friday 8:00 AM-11:15 AM and 12:45 PM - 4:00 PM.
All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Classroom 1A / 1B: Staff: S2, S3, S4, S5, S6 with 20 children; Classroom 2: Staff S7 with 5 children; Classroom 3: Staff: S8 with 5 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. All children were observed to be under visual supervision of a teacher at all times.
The purpose of this visit is to conduct a follow up to the self reported Unusual Incident dated 06/13/23. During the inspection LPA interviewed staff members, reviewed records and obtained documents (roster, parent handbook, swim lessons notification). During the staff and Director interviews, all indicated that the bus driver had control of the bus door by use of a button located on the left side of the driver's seat. Transportation procedures were explained by staff and Director as having a teacher on the bus first to help assist children to their assigned seats. Another teacher stands on the outside of the bus, by the door, assisting children onto the bus and up the steps. Teachers and staff are staggered through out the bus to ensure 100% supervision at all times during the trip. Director states that she has never had an Unusual Incident occur during her tenure as Director and was unaware that she would need to report the incident directly. Director further states the since the incident, the Regional Manager has discussed reporting requirements procedures with all Directors within her covered Region. LPA also discussed reporting requirements for an unusual incident during the inspection.
