13 | Licensing Program Analyst (LPA) Susann Sanchez arrived at the above facility to deliver findings for the allegations above. LPA Sanchez told Licensee A. Tamrat the purpose of the visit and was granted entry into the facility and given a tour. Census was taken. Upon arrival, LPA observed an electrician outside the facility installing cameras and left at 2:19pm. Electrician was not near children in care.
During the investigation, LPA Sanchez conducted interviews with the Licensee, staff (2), resident, children (3), and parents (7) and there was no corroborating statements made during interviews. Reporting Party (PR) was interviewed, there were some discrepancies with the timeline and original statements when allegations were first reported to the department. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and a copy of the report and appeal rights were provided to the Licensee A. Tamrat.
