Licensing Program Analyst (LPA) Franchesca White arrived at the above facility for the purpose of an unannounced Lead Exceedance Case Management visit. LPA White announced the purpose of the visit and was granted entry into the facility by Program Director Mildred Lovett. There were 25 children present (Room 5: 10 day care children, and two staff; Room 6: 3 day care children, and 1 staff; Room 7: 5 day care children, and 2 staff; Room 8: 7 day care children, and 2 staff. All staff have current fingerprint background clearance.

Today's visit was to clear the POC for the Case Management: Lead Exceedance visit from May 5, 2023. The testing sites that were observed were Water Faucet "B" located in Room 3, and Water Faucet "E" located in Room 6. Upon entry of Room 3, LPA White observed the room to be unoccupied by children or staff. Program Director states that the room has not been used since 2020. LPA White observed water faucet "B" to be made inaccessible with a white plastic covering tightly sealed over the faucet with a heavy adhesive. Director Lovett removed it for LPA to observe the faucet nozzle being turned with "no water" dispensing from the faucet. Per Director, water valves have been shut off making water source in accessible to children. Furthermore, room is locked making it inaccessible to children in care.

LPA White observed water faucet "E" located in Room 6. LPA White found the room to be occupied by 3 children napping, and one staff member. Faucet "E" is made inaccessible with a white plastic covering tightly sealed over the faucet with a heavy adhesive. Director Lovett removed it for LPA White to observe the faucet nozzle being turned with "no water" dispensing from the faucet. LPA observed bottles of water for each child labeled. Per Director, the school provides water for the children.

LPA White discussed further means of action to make faucets permanently inaccessible by removing the new faucet heads from the sink. Per Director, she will have the faucets retested for lead, but intends for the water to remain shut off. Per Director she will provide a copy of the lead testing results to LPA White by March 25, 2024. ................................Report Continues..............1 of 2 Pages..............................................................

Program Director had proof of lead results, and notification. Program Director provided LPA with a copy of the proof of the plumbing company that repaired faucets and coverings.

Today's visit will clear the Plan of Correction, and further documentation will be added to the report upon receipt from Director Mildred Lovett.

Based on observations, record review, and interviews there are no deficiencies to be sited today.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
Exit interview was conducted and report was reviewed with the Program Director, Mildred Lovett. A copy of the report, Notice of Site Visit, and Appeal Rights were given.
