32 | Regarding the allegation: Injuries due to lack of supervision. Reporting Party (RP) alleged that Child #1 (C1) was injured multiples times due to lack of supervision. Although staff to child ratios are met, there are two teachers assigned to care for 16 children. Some of those children are not fully potty trained and require diapering. Interviews indicate that while one staff member is occupied with diaper changes, the other staff is not always able to adequately supervise the rest of the children to prevent injury. Staff interview also revealed that some children were bit and turning over cabinets. LPA conducted a file review for C1 which found that there were multiple incident reports between 09/06/24 through 10/07/24.
Based on LPA’s record review and interviews which were conducted, the preponderance of the evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 & Chapter 1, Article 06, 101229(a) is being cited on the attached LIC9099D.
An exit Interview was conducted, a copy of this report and appeal rights along with Notice of Site visit was provided to the Preschool Site Supervisor. Notice of Site visit must be posted for 30 days.
