On Wednesday, May 3, 2023 at 2:06 PM, Licensing Program Analysts (LPA’s) Mayra Rivera and Warren Birks conducted an announced inspection at the facility noted above and met with Director, Jennifer Hollandsworth. The purpose of the inspection was to conduct the required inspection for a pending Capacity Change. This facility is currently licensed for 105 preschool children and requested to increase their capacity by adding 12 toddlers for a total capacity of 117; 105 preschool children and 12 toddlers. This facility consists of 6 classrooms: Room 1, Room 2, Room 4, Room 5, Room 6, and Room 7 (toddlers). The operating hours for this facility is Monday through Friday from 6:30 AM to 6:30 PM. A fire clearance has been granted on January 26, 2023, for a total of 117 children. This facility also has a licensed infant center located on-site (License #: 198005945

At approximately 2:10 p.m. LPA’s entered the preschool classroom 1 and observed a total of 17 children with staff #1 and staff 2. LPA’s observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA’s observed cubbies with children’s personal items. LPA’s observed cleaning solution stored inside high top cabinets making it inaccessible for children to reach. LPA’s observed a step trash bin with a tight-fitting lid.

At approximately 2:15 p.m. LPA’s entered the preschool classroom 2 and observed a total of 19 children with staff #3 and staff #4. LPA’s observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children’s personal items. LPA observed cleaning solution stored inside high top cabinet making it inaccessible for children to reach. LPA’s observed a step trash bin with a tight-fitting lid.

At approximately 2:23 p.m. LPA’s entered the preschool classroom 4 and observed a total of 22 children with staff # 5 and staff #6. LPA’s observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. At approximately 2:29 p.m. LPA’s entered the preschool classroom 5 and observed total of 22 children with staff # 7 and staff #8. LPA’s observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA’s observed cubbies with children’s personal items. LPA observed cleaning solution stored inside high top cabinet making it inaccessible for children to reach. LPA’s observed a step trash bin with a tight-fitting lid.

At approximately 2:34 pm. LPA’s entered the preschool classroom 6 and observed a total of 12 children with staff # 9. LPA’s observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children’s personal items. LPA’s observed cleaning solution stored inside high top cabinet making it inaccessible for children to reach. LPA’s observed a step trash bin with a tight-fitting lid.

At approximately 2:40 pm. LPA’s entered classroom 7 (toddlers) and observed the classroom not being utilized. This is the classroom that is being added to the capacity increase for 12 toddlers. LPA’s measured the classroom and observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA’s observed changing table arm’s length to handwashing sink. LPA’s observed cubbies for children’s personal items. LPA’s observed cleaning solutions stored inside high top cabinet making it inaccessible for children to reach. LPA’s observed a step trash bin with a tight-fitting lid.

LPA’s observed the bathrooms located between classroom 1 and 2 ( 3 toilets and 4 sinks), classroom 4 and 5 (3 toilets and 4 sinks). Classroom 6 (1 toilet and 1 sink) and 7 (2 toilets and 3 sinks) bathrooms are located inside the classroom. LPA’s observed a total of 9 toilets, 1 changing table, and 12 handwashing sinks. LPA’s observed the bathrooms to be in good condition with working toilets, sinks, hand soap dispenser and paper towels. Ill/isolation area children utilize the staff restroom located in the front office. Floor mat will be utilized for toddlers in diapers.

At approximately 2:50 p.m. LPA’s entered the toddler outdoor playground to conduct an inspection and measurements. LPA’s observed the toddler yard to have a grassy area and fenced all around. LPA’s observed the surface of the outdoor activity space maintained in a safe condition and is free of hazards. LPA’s did not observe climbing structure equipment. LPA’s observed age appropriate play equipment and area to be in safe condition, free of sharp, no loose or pointed parts. LPA’s advised to add more outdoor play equipment. For outdoor drinking water, was observed (water fountains, pitchers, and disposable cups). For adequate shade, LPA’s observe a four-post triangle shade structure.

Heating lighting and ventilation were evaluated by LPAs. LPA’s observed central AC/heater and vents located on the ceiling. LPA observed the fire extinguishers located inside the classrooms and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 2/3/2023. Smoke detectors were not tested during this visit due to the detectors being wired to the emergency pull down fire alarm. Carbon monoxide detector was tested and is operable. LPA observed napping equipment to be in good condition. Per director, the isolation area is located in the office and a cot can be placed for an ill child. First Aid supplies are kept inside each classroom and at the front desk. Per director, medication is only administered to a child when accompanied with a doctor's note and medication is stored inside the classrooms and/or office. LPA Rivera asked if there are any poisons, firearms, weapons or bodies of water. Director stated she has no firearms, weapons, no poisons nor bodies of water. LPA did not observe firearms or weapons, poisons, nor bodies of water.

This program provides breakfast, a.m. snack, lunch, p.m snacks. Meals are prepared on site. A monthly menu is available and posted in the main entrance and in preschool classroom. For water drinking, facility provides a water pitcher and disposable cups and children bring their own sippy cups. LPA Birks observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents.

The following documents were posted in a prominent, publicly accessible area at the facility: Facility License, Notification of Parents' Rights (PUB 394), Personal Rights (LIC 613A), Menu, and Earthquake Preparedness (LIC 9148).

Director Jennifer Hollandsworth has pediatric First Aid/CPR certification dated 5/27/22, preventative health and safety training dated 7/30/2017, child abused mandated certification (AB 1207) expired 7/8/24 and has proof of immunization against pertussis, and measles, and influenza declination. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA advised the facility representative how to access forms, regulations, quarterly updates, and Provider Information Notices (PINs) on the Department website at: www.ccld.ca.gov.

Based on measurements taken during today's inspection and availability of toilets and sinks, the facility is equipped for the capacity increase of adding the 12 toddlers to the preschool license.

A final review of the application will be conducted by the Department. Once licensed, the Licensee is required to adhere to the terms and limitations stated on the license.

No deficiencies were cited during today's inspection. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with Director, Jennifer Hollandsworth.
