Licensing Program Analyst (LPA) R. Derraco conducted an unannounced complaint inspection to the above mentioned facility on 04/30/26 at 11:00 AM. LPA was met by Director, Monica Gonzalez, who guided analyst on a tour of the facility. LPA observed 12 staff members and 89 children in care. The center's pre-school license has a toddler component. The physical plant of the center was observed to be in good repair.

The purpose of this visit is to deliver findings to the above mentioned facility. During the investigation, LPA conducted interviews, reviewed records and made observations. Individuals interviewed stated that Staff (S)3 observed S5 and S6 lock Child (C)1 in a closet as a form of punishment around the month of January or February 2026. During an interview with S5 on 05/12/26, they did not confirm or deny that the incident took place. S5 stated, “I am not going to lie but I don’t want to say anything because…”(no further comment). that was obtained by the center during a separate incident that occurred on 02/20/26, S4 explained that she was advised by S3 of the incident and was told that "Moncia is aware." Per Director, she has no knowledge of the incident. Based on LPA interviews conducted, the preponderance of evidence standard has been met,therefore the above allegation is found to be SUBSTANTIATED. A citation under California Code of Regulations, Title 22, Division 1 and Chapter 12 Section 101223(a)(3) are being cited on the attached.

LPA R. Derraco informed Director, Monica Gonzalez that this report dated 04/30/26 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA R. Derraco informed the Director, Monica Gonzalez, to provide a copy of this licensing report dated 05/12/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.
