Licensing Program Analyst (LPA) R. Derraco conducted an unannounced complaint inspection to the above mentioned facility on 04/30/26 at 1:00 PM. LPA was met by Director, Monica Gonzalez, who guided analyst on a tour of the facility. LPA observed 12 staff members and 89 children in care. The center's pre-school license has a toddler component. The physical plant of the center was observed to be in good repair.

The purpose of this visit is to deliver complaint findings to the above allegation. During the course of the investigation, LPA conducted interviews and reviewed staff records. S1 disclosed to LPA that an incident occurred in Room 7 where a child was left unattended in the outdoor play area of the center. Individuals interviewed stated that the child was able to sneak past a teacher during a classroom transition from outdoor playtime to indoor classroom time. Individuals state that the teachers noticed the child was not in attendance when roll was taken after the children had come inside of the classroom. S1 stated that a parent had informed them that there was a child left unattended in the outdoor play area. S1 stated that she immediately attended to the child and brought the child back into the classroom. Individuals interviewed

stated that a parent was speaking to one of the teachers during the transition from outdoors to indoors which led to the teacher being distracted enough not notice a child walking out the door into the outdoor play area. Individuals interviewed state that the child was alone for approximately 1 minute and up to 10 minutes. Regardless of the time left alone, the child was not visually supervised by a teacher. LPA observed that the outdoor play yard has perimeter fencing. Based on LPA's observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. LPA advised Director that a Type A citation under California Code of Regulations, Title 22, Division 1 and Chapter Number 12, Article 6, Section 101229(a)(1) will be issued on the attached LIC 9099D

LPA R. Derraco informed Director Monica Gonzalez that this report dated 04/30/26 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA R. Derraco informed the Director Monca Gonzalez to provide a copy of this licensing report dated 04/30/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

Director states will have name to face transition re-training with current staff. Director will create a sign in sheet to indicate specific training taken by staff. Director will email copy of sign in sheet to LPA by next business day. Training will be held on 05/21/26 with all staff.
