32 | stated that a parent was speaking to one of the teachers during the transition from outdoors to indoors which led to the teacher being distracted enough not notice a child walking out the door into the outdoor play area. Individuals interviewed state that the child was alone for approximately 1 minute and up to 10 minutes. Regardless of the time left alone, the child was not visually supervised by a teacher. LPA observed that the outdoor play yard has perimeter fencing. Based on LPA's observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. LPA advised Director that a Type A citation under California Code of Regulations, Title 22, Division 1 and Chapter Number 12, Article 6, Section 101229(a)(1) will be issued on the attached LIC 9099D
LPA R. Derraco informed Director Monica Gonzalez that this report dated 04/30/26 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA R. Derraco informed the Director Monca Gonzalez to provide a copy of this licensing report dated 04/30/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days.
