Licensing Program Analyst (LPA) R. Derraco conducted an unannouced complaint inspection to the above mentioned facility on 06/12/26 at 11:15 AM. LPA was met by Director, Monca Gonzalez, who guided analyst on a tour of the center. During the tour, LPA observed 83 children in care and 10 staff members caring for children. The physical plant of the center was observed to be free of defects.

The pupose of this visit is to deliver findings to the above mentioned facility. During the course of this investigation, LPA conducted interviews and made observations. Individuals interviewed stated that C1 has had their arm pulled by S1 and S2. Individuals interviewed also stated that C2 has had their arm pulled by S1 and S3. Individuals interviewed stated that when C1 and C2s arms were pulled, that it would hurt. individuals interviwed stated that S1, S2 and S3 would pull C1 and C2s arms because they were misbehaving. Based on LPA's interviews conducted, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations, section 101223(a)(3, are being cited on the attached LIC 9099D.

A violation regarding California Code of Regulation (CCR) Section 101223(a)(3) warrants an immediate civil penalty of $250.00 . A repeat violation civil penalty is assessed because the licensee was cited for violating the same provision within the previous 12 months.

LPA R. Derraco informed director Monica Gonzalez that this report dated 06/12/26 document(s) one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA R. Derraco informed the director Monica Gonzalez to provide a copy of this licensing report dated 06/12/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Notice of site visit was given and must remain posted for 30 days.

Licensing Program Analyst (LPA) R. Derraco conducted an unannouced complaint inspection to the above mentioned facility on 06/12/26 at 11:15 AM. LPA was met by Director, Monca Gonzalez, who guided analyst on a tour of the center. During the tour, LPA observed 83 children in care and 10 staff members caring for children. The physical plant of the center was observed to be free of defects.

The purpose of this visit is to deliver findings to the above mentioned allegations. During the course of the investigation, LPA conducted interviews and made observations. individuals interviewed state that they do not have any concerns with how staff members treat children in care. During previous visits, LPA observed classrooms 4 and 5 to have the most children in care. Staff members would often elevate the volume of their voice to get the attention of the classroom. Individuals interviewed did not disclose any injuries sustained by children in care, while in care. LPA did not observe any Unusual Incidents reported by the center to corrobarate that children sustained any injury. Although the allegation my have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the
