Licensing Program Analyst (LPA) R. Derraco conducted an unannounced complaint inspection to the above mentioned facility on 04/30/26 at 9:30 AM. LPA was met by Director, Monica Gonzalez, who guided analyst on a tour. LPA observed 13 staff members caring for 41 infants. The center's physical plant was observed to be in good repair.

The purpose of this visit is to deliver findings to the above mentioned allegation. During the course of the investigation, LPA conducted interviews, reviewed records, made observations, and obtained documentation. Individuals interviewed confirmed that S2 was seen striking S6 in the head with a children's toy (rattle/morraca). Individuals stated in an interview conducted by the center, that S2 was struck by S6 by the children's toy. In retaliation, S2 struck S6 in the head with the same toy saying "look it hurts, these are meant for shaking. Assistant Director, Vivian Franco confirmed that the incident took place by self reporting an Unusual Incident on February 23, 2026. stated that there were previous incidents where S2 would hit S6 on the back of the neck as a form a discipline for hurting other individuals. Assistant Director stated that S2 was placed on administrative leave after the incident was reported to management. Based on LPA's interviews conducted and the center's records reviewed regarding this incident, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. LPA advised Director that a Type A citation under California Code of Regulations, Title 22, Division 12 Chapter 1, Article 6, section 101223(a)(3) will be issued on the attached LIC 9099D.

LPA R. Derraco informed Director Monica Gonzalez that this report dated 04/30/26 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.

Also, LPA R. Derraco informed the licensee to provide a copy of this licensing report dated 04/30/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

Director states that S2 was terminated from employment. Director also states that a meeting will be held with staff no later than 05/22/26 to discuss this issue. Director states will send LPA a sign in sheet of employees who have attended.
