On May 31,2024 at 9:30AM, Licensing Program Analysts (LPAs) Jonnisha Culbert and Jeanette Estrada conducted an unannounced inspection at the facility noted above and met with Licensee, Kathryn Centanni. The purpose of the inspection was to conduct an Annual/Random inspection. The operating hours of the facility is Monday through Friday from 7:00 AM to 5:00 PM. Entrance Checklist (LIC 126) was provided to the Licensee upon arrival. Individuals residing in the home were discussed and noted. At the time of the inspection, the Licensee, Staff 1, Adult 1, and Adult 2, and ten (10) children (four infants) were present.

This facility is a single family home that consists of four (4) bedrooms, two (2) bathrooms, living room, kitchen, den, detached garage, and fenced backyard. Per Licensee, areas off-limits to children include: 4 bedrooms and 1 bathroom near the den. On limits areas consist of living room, bathroom near the living room, kitchen, den, and backyard. Per Licensee, garage located in backyard is currently being reorganized and because of that for the past two weeks she has not taken children in care outside to play in backyard.

Areas currently used by children were inspected for safety, comfort, heating, cleanliness and telephone service. Living room was observed to have appropriate toys and materials for children. Children nap in the living room. LPAs observed napping cots and 3 play yards available. Although there are four infants present, Per Licensee one infant resides in the home and the infant stays with parents in off limits area of the home during operation hours. Per Licensee, infant sleeps in a bassinet in the room with parents. Licensee was advised that when infant is present in the home they count towards capacity. In the kitchen, LPAs observed knife block and items on counters. LPAs advised Licensee to clear all items on the counter that may pose a potential safety risk and make knives more inaccessible by placing them in a cabinet above counter top. LPAs advised Licensee to install safety knobs on stove for added safety. Licensee stated that meals are prepared for children in care. Licensee was reminded that food that is brought from the child's home shall be labeled with the child’s name and properly stored or refrigerated.

LPAs observed the required fire extinguisher (2-A:10-B:C) that is fully charged. LPAs observed the fire extinguisher on the wall by window in the den between the kitchen in the den with an attached receipt
dated 02/11/2024. Licensee was reminded to have the fire extinguisher serviced yearly.
LPAs observed an operable smoke detector located in the hallway near on limits bathroom and an operable carbon monoxide detector located on top of cabinet in dining room. In the Den, LPAs observed a fireplace (not in use) with a glass door fitted for the fireplace. LPAs informed the Licensee that the glass door attached to the fireplace could get hot while the fireplace is in-use. Licensee stated that fireplace is never used when children are in care and that the den is only used through passing to escort children to the door that leads to the gated backyard area.
Detergents, cleaning compounds, and medicines were made inaccessible to children. Cleaning compounds are kept in a cabinet above the refrigerator and in the hallway above washer and dryer. Per License, medicines are kept above washer and dryer too. Per Licensee, currently no children in care require medication. Per Licensee, there are no poisons kept in the home. Licensee was advised that if any poisons are purchased, it is required to be locked and in an inaccessible area. Per Licensee, there are no firearms and/or weapons on the premises.
On limits restroom was inspected for safety. LPAs did not observe any hazards in the restroom.

In the backyard, LPAs observed scattered toy structures, boxes, cots, and 3 packed playpens. Also in the backyard, LPAs observed a water well/fountain located in a flower bed. Licensee was advised to turn of fountain and remove water when children are in care. Per License, grassed area is used in the summer time and cemented area is used in the winter. Per Licensee, a gate is used to prevent access to the grassed area of yard during the winter. Licensee stated that she has 2 cats and a dog.

LPAs conducted a record review of 9(nine) children's records, Staff record, and the Licensee's record. Based on the children's record review all required documents were observed in folder. Based on Staff record review, measles and TB clearance were not observed. Staff has current Pediatric First Aid and CPR Certification (08/2025) and Mandated Reporter Training (completed on 11/15/2022). LPAs observed all required documents for Staff. Based on Licensee's record review, Licensee's proof of immunization against measles, pertussis, influenza, and a TB clearance were observed. Licensee has a current Pediatric First Aid and CPR certification (expires 8/2025), and Mandated Reporter Training completed on 11/14/2022. Based on interview, TB clearance for Adult 1, Adult 2, Adult 3 and Staff was not available. This poses a potential threat to the health and safety of children in care.

Currently Licensee does have infants enrolled. LPAs discussed the safe sleep regulations with the Licensee and discussed the child care licensing safe sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
LPAs also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a plan for providing IMS must be submitted to the Department. The following information regarding

Licensee was informed of the Mychildcareplan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and resource and referral agencies (R&Rs) throughout California.
During the exit interview, the Licensee, Kathryn Centanni confirmed that there are no Registered Sex Offenders living in the facility and LPAs completed the RSO in FAS.
California Code of Regulations, (Title 22, Division 12, Chapter 1), are being cited on the attached LIC 809 D

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
