25 | On 07/07/2026, a case management incident inspection was conducted by Licensing Program Analyst (LPA) M. Do who met with Director Wendy Atristain. Director was informed of the purpose of visit. Upon arrival there were 36 preschool children and five (5) staff. Director stated that currently there are 68 children enrolled.
A review of the Facility Personnel Report Summary on this date 07/07/2026 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
A self-reported incident by the facility was received at the regional office on 6/26/26, that on 6/22/26, a child held his breath and fainted. He regained consciousness prior to the arrival of paramedics.
LPA interviewed one (1) staff, the senior director, the director, and Child 1 (C1). The senior director stated, by Friday 07/17/2026, they will request from parents the physician’s instructions on how to handle similar future incidents. The instructions will be kept in the child’s file and shared with the staff to ensure appropriate intervention and safety of the child.
Further information is needed if any deficiencies are observed, disclosed, or discovered during today’s visit, they will be addressed and cited at a later date.
Exit interview was conducted with Senior Director Atristain. Report was reviewed and discussed. Notice of Site visit was provided and must remain posted for 30 consecutive days. Appeal Rights were provided.
