25 | On April 22, 2024, at 9:00 am Licensing Program Analyst (LPA) Carolyn Tuba conducted a Case Management inspection due to an incident that allegedly occurred at the facility on 03/08/2024. A Covid Risk assessment was conducted. LPA met with Staff, Socorro Mata who guided LPA on a tour to obtain the census of 11 children with 2 staff, as well as an in additional adult was present conducting observations during the visit. Child Development Supervisor, Mercedes Gomez arrived at the facility at 9:30 am who works offsite and supervises this facility.
The incident was reported to the Department within the required 24 hours of occurrence. The incident consisted with a child disclosing to their parents that the staff had violated their Personal Rights.
Based on all the interviews conducted with the Child Development Supervisor (CDS), Staff #1(S1), Staff #2 (S2) and Child#1 (C1), LPA found that there were no disclosures made that C1's personal rights were violated. The facility conducted their own investigation and did not discover staff behaving inappropriately with the incident reported by C1’s parents. C1 continues to attend the facility and parents of C1 have not communicated any further with staff or administrators after the initial disclosure. The administration stated that they have made several attempts to hold a meeting with parents of C1, but they have not responded.
No deficiencies were cited during today’s inspection.
Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Child Development Supervisor, Mercedes Gomez.
