On 10/22/2024 at 9:47 AM, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced inspection at the facility noted above and met with Licensee, Lucinda Hawkins. The purpose of the inspection was to conduct an annual/ random. The operating hours of the facility is Monday through Friday from 6:00 AM to 6:00 PM. Entrance Checklist (LIC 126) was provided to the Licensee upon arrival. Individuals residing in the home were discussed and noted. At the time of the inspection 3 (three) dogs, the Licensee, assistant 1, and assistant 2, and ten (10) children were present.

All areas on the first floor of the facility were inspected. This facility is a two-story family home that consists of seven (7) bedrooms, four (4) bathrooms, family room, living room, kitchen, detached garage, bonus room (attached to garage), gated driveway, and fenced backyard.

Areas that are accessible to children include: two (2) bedrooms (located downstairs), 2 bathrooms (located downstairs), family room, living room, kitchen, bonus room, gated driveway, and fenced backyard. Per Licensee, the children utilize the gated driveway and fenced backyard for outdoor activity. Per licensee, children eat and sometimes sleep in the bonus room and a copy of the permit for the bonus room is on file with the Department.

Areas off-limits to children include: four (4) bedrooms 4 (located upstairs), 2 bathrooms (located upstairs), and garage. At 10:15 am, LPA did not observe a child safety gate or barrier installed, making the stairs accessible to children. LPA also observed the off-limit bedroom door located downstairs was unlocked, making the room accessible to children in care. During the inspection, Licensee installed a child safety gate to the stairs. LPA advised licensee to make off limits area inaccessible to children by locking the door. Licensee was advised that off-limit areas must be made inaccessible during operating hours or while children in care are present.

The following documents were posted: Facility License, Notification of Parents' Rights (PUB 394), Earthquake Preparedness (LIC 9148), and Emergency Disaster Plan (LIC 610A). Licensee has a current Facility Roster (LIC 9040). Licensee also has verification of disaster and fire drills.

Areas used by children were inspected for safety, comfort, heating, cleanliness, and telephone service. The home does not have any wall heaters or fireplaces. The home has central air and heating. Detergents and medicines were made inaccessible to children. Per Licensee, poisons and cleaning compounds are kept in the off-limits garage. LPA observed that the garage door was not locked making garage accessible to children in care. LPA informed licensee of the regulation for storing poisons and cleaning compounds and advised licensee to lock garage door while children are in care. Per Licensee, there are no bodies of water, firearms and/or weapons on the premises. LPA observed age-appropriate toys and napping equipment for children. LPA did not observe electrical outlet covers installed in the childcare areas.

At 10:35am, LPA observed knives kept in the dish washer. Per licensee, they keep the knives in the dishwasher. LPA advised licensee to make knives inaccessible by moving them out of reach of children.

At 10:30am, licensee asked LPA about a recall on two swings that they have. Per licensee, they were recently made aware of the recalled swings, and they have not used the swings in a while. LPA confirmed that the swings are recalled devices. Licensee instructed assistant to contact the city to pick bulky trash (swings).

LPA observed the required fire extinguisher (2-A:10-B:C) that is fully charged and was purchased on December 17,2023. Licensee was reminded to have the fire extinguisher serviced yearly. Smoke detectors and carbon monoxide detectors were tested and are operable. First Aid kit and emergency supplies are available and kept in a bedroom located downstairs. Licensee provides breakfast, lunch, dinner, am and pm snacks to children in care. Licensee was reminded that if food is brought from the child's home shall be labeled with the child’s name and properly stored or refrigerated.

LPA conducted a record review of 5 children's records, Licensee's record, Assistant 1 record, and Assistant 2 record. Based on record reviews, children files, licensee and assistants records were all complete. LPA observed that Licensee and assistants had current Pediatric First Aid and CPR certification, current Mandated Reporter Training certificate, and immunization records. Licensee was advised that the Mandated Reporter training (AB 1207) must be completed every 2 years and is available at www.mandatedreporterca.com.

Licensee Lucinda Hawkins was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee Lucinda and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee Lucinda of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee Lucinda was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Lucinda Hawkins, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

No deficiencies were cited during today’s visit.

A notice of site visit was given and must remain posted for 30 days.
