On 12/12/2023 LPA Whitmore initiated the complaint investigation and met with the Site Supervisor Donna Belvin. LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with a total of 7 children and 3 teachers. LPA interviewed the Site Supervisor & Staff. LPA obtained a copy of Personnel Report, Facility Roster, Infant Schedule, Polices & Procedures, Staff Development Agenda & Personnel Summary from teachers How not to violate Children's Personal Rights. LPA conducted observation during outdoor activities.
For the purpose of complaint investigation LPA Whitmore interviewed the Site Supervisor and Staff, based upon interviews the allegations need further investigation.
On 12/14/2023 at 4:18 p.m. Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced complaint investigation and met with Donna Belvins. LPA explained the purpose of the visit to continue interviews with the Staff. LPA toured the facility indoors and outdoors and observed 3 children and 3 staff. Based upon interviews the allegations need further investigation.

On 02/21/2024 at 9:08 a.m. LPA Whitmore conducted a visit to complete the investigation and deliver findings. LPA Whitmore met with the Site Supervisor Donna Belvin. LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with 8 total children in care and 7 Teachers. The Department conducted a full investigation, which included staff interviews, interviews with relevant parties, as well as a record review and observation. LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of Lack of Care & Supervision - Staff did not ensure child was provided adequate care and supervision, Personal Rights - Staff did not ensure child was spoken to in an appropriate manner, Personal Rights - Staff does not ensure child eats snacks on a clean surface while in care. Although the allegations may have happened or are valid there is not a preponderance of evidence to prove the alleged violations did or did not occur. Therefore, the allegations are deemed unsubstantiated. An exit interview was conducted, a copy of this report, appeal rights along with Notice of Site Visit were provided.
