Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection on 07/18/25 due to an incident that occurred on Thursday, 07/10/25. LPA arrived at the facility at 9:10 AM and met with Deidre Corwin, Program Director who guided analyst on a tour of the facility. This is a preschool program which consists of 4 classrooms: Room 101, Room 104, Room 105, and Room 108. Effective 07/14/25 this program is closed until January 2026. During this inspection no children were present on site. There are school age programs and Elementary program on this site. All the programs are under Glendale Unified School District.

Alleged Incident took place on Thursday 07/10/25. The incident was reported via email to the department on Friday 07/11/25. The facility reported the incident within the required 24-hour time frame.

LPA Eivazian conducted an interview with staff#1. LPA watched facility video footage for 07/10/25 from 2:26:56 p.m. to 2:35:46 p.m. from facility camera 5 on staff#1 laptop. During this inspection Deidre Corwin, Program Director emailed LPA Eivazian 07/11/25 video footage and program director internal investigation notes.

Based on an interview that was conducted with staff#1, on 07/10/25 at the time of the incident there were total of 8 children present in Room 105. Seven children transitioned from Room 105 to 104 at 2:26:56 p.m. , with 2 staff, staff#2 and staff#3. At 2:27:27 p.m., staff#2, entered Room 105, carrying her purse and closed the door behind her. Her scheduled workday ended at 2:30 p.m.. At 2:31 p.m., a parent entered Room 105 to retrieve their child’s belongings. The room was dark, based on the video footage. Child#1 in the restroom was not seen or heard. At 2:35:46 p.m. staff#4 entered Room 105 to begin her break. Upon hearing running water, staff#4, investigated, the Room 105 restroom door was not closed, it was a little bit open and found child#1, alone in the Room 105 dark restroom flushing second toilet which is close to Room 108 and stated, “the teacher left me going potty”.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Per staff#1, staff#4 checked on the child#1 and child#1 was unharmed. Per staff#1, child#1 is really verbal and clearly stated to mother the teacher left child#1 in the bathroom alone.

Per staff#1, facility has been closed effective 07/14/25 till January 2026. Teachers are getting training to be ensure they are ready prior reopening of this site.

Based on LPA observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been meet, therefore the above incident that occurred on 07/10/25 and child#1 was unsupervised, alone and left unattended in Room 105 bathroom from 2:25:56 p.m. to 2:35:46 p.m., for 9 minutes is found to be Substantiated at this time. California Code of Regulations, 101229(a) (1)-- Responsibility for Providing Care and Supervision--(a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation and Personal Rights-- 101223(a)(2)--Per The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs are being cited on the attached LIC9099D.

LPA Anomeh Eivazian informed Program Director, Deidre Corwin that this report dated 07/18/25 documents 2 of Type A citations. Type A citations which shall be posted for 30 consecutive days as there is immediate risks to the health, safety, or personal rights of children in care.

Also, LPA Anomeh Eivazian informed the Program Director, Deidre Corwin to provide a copy of this licensing report dated 07/18/25 that documents 2 Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.
