25 | Licensing Program Analyst (LPA), Anomeh Eivazian, conducted an unannounced case- management inspection to the above facility on 02/20/25 at 8:50 am. LPA met with Site Director, Juny Gray who guided LPA on a tour of the facility. At 10:20 AM Perla Leyvas, Program Supervisor arrived to the facility. During this inspection there were total of 50 children present in the facility with 12 staff.
Based on interviews that were conducted with four staff on 02/07/25 and video footages that were watched by LPA Eivazian and staff#3 on 02/07/25 at the school office area, child#1 was observed to be unsupervised and wondered away alone at school age enclosed yard from 4:18:34 PM till 4:22:25 PM on 01/31/25 and this incident was not reported to licensing in a timely manner.
Alleged Incident took place on 01/31/25. Incident was reported to the Department via email on 02/10/25. The facility did not report the incident within the required 24 hour time frame.
The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
